6 Purpose of Investment Review Form To clearly outline the intent and contents of investment projects and avoid confusion between the different Church Leadership groups. To perform a review for Equipment and Software costing more than $2500 with a life greater than one year. The form should be prepared by Project Leader and signed by: –Pastor (Overall authorization as head of Church) –Director of Operations (ensure alignment with strategic objectives (internal & external), Church needs, and general plant operations) –Trustee Board Chair (ensure availability of funds and connections to budget) –Maintenance Manager (ensure equipment are of the right specifications, conforms to safety requirements, codes, integration with existing facilities, etc.) Projects should be reviewed with Trustee Board so they can be aware of them and can plan funding. No Trustee vote is required for projects that were in the budget approved by the Church. However, new projects will require a vote by Trustee Board. The Director of Operations will review with all signees ahead of presentation to Trustee Board. Either the Project Leader, Dir. of Operations or Trustee Chair can present to Trustee Board.

Instructions Estimated Cost –The total amount required to complete the project regardless of timing of the expenditure. A supplementary form can be completed later if the final estimates exceed the initial estimates. Project Description/Scope –Describe the project and what it entails. Briefly explain why we need it. Project Financials/Benefits –Show the timing of cash outlay and any savings that will be received from project. If savings are not quantifiable, explain in words any benefits that will be received. Project Timing/Milestones –Show major milestones and timing. Start with when the first activity will be completed and end with the time when the project will be completed. Be as specific as possible so that the stakeholders will know when utilization will commence. Risks/Issues –List any circumstances that could adversely affect the project. Outline mitigation or contingency actions that will help keep project on schedule and deliver desired results. Details –List as much details as possible about the project. Show list of equipment and steps that will be taken to implement project. Documentation such as quotes from vendors, drawings, etc. can be attached. Signature –Signature of Church Leadership as designated on previous page 7
