The requestor is required to follow the lab’s policy and guidelines to ensure that the security and safety measures are adhered. In the event where the manager decided to pass the key to the requestor due to after office hours or weekend event, the requestor will be required to fill up the Key Request Form and obtain approval from the Dean / Deputy Dean. Key Request Form 9The approved form shall be submitted to the Manager/Lab Engineer or the authorized technician when the requestor collect the key. Manager/Lab Engineer/Technical Assistant/Technician shall sign the form and record in the logbook. The form shall be filed by the Manager/Lab Engineer/Technical Assistant/Technician. If the event is beyond the office hour or working day, the Lab Engineer or the authorized technician shall check the lab before the event to ensure it is in good condition. The Lab Engineer or the authorized technician shall also check the lab after the key is returned. 10, 11The key must be returned to the office the next working day after the event.
