FHIR – DICOMweb use cases F. Behlen IIWG/WG20 Tuesday Q3

Up-Front Conclusions/Biases DICOM image display requires knowledge of the Standard For Display of DICOM data, FHIR is not needed Viewers for DICOM images already exist DICOM vendors won’t implement it for nothing DIMSE and DICOMweb services already standardized FHIR users won’t pay what it costs Best use of FHIR will be for retrieval of DICOM-encapsulated documents and non-dicom images STOWed in DICOM Some EHRs assume only one viewer. Others may allow multiple viewers. This is the more sustainable solution

Traditional Picture “Universal” Viewer Multiple viewers URL URL EHR with accnum as exam context EHR with accnum as exam context EHR with URL as exam context AccNum AccNum URL RegEx RegEx URL URL Viewer Universal Viewer Picture viewer Specialty Viewer DICOM 2D Viewer DIMSE or DICOMweb DIMSE or DICOMweb DIMSE or DICOMweb PACS (or VNA) Other image storage PACS (or VNA) Other image storage PACS (or VNA)

What about “informal” imaging? STOW avoids DIMSE, but still has DICOM content constraints Tablets Mobile devices FHIR Resources may be a way to ease these constraints for informal imaging. But then we don’t need to spend a lot of time fleshing out the DICOM mapping for FHIR General Market imaging & Video tools Others who just don’t want to learn about DICOM

A Cautionary Tale CDA R2 contains a set of graphical resources, regionOfInterest, for annotation of DICOM images Developing these required a lot of time, negotiating with other TCs about such things as reconciling DICOM’s pixel-edge based coordinate system with (pixel-centered) row-column addressing in general imaging tools. When it came to profiling the CDA Diagnostic Imaging Report, regionOfInterest was never used, instead using GSPS because we concluded that the rendering process was more safely handled by DICOM-aware rendering applications.

Study Query FHIR Imaging Study QIDO Datatypes consistent with other FHIR resources Documentation consistent with other FHIR resources Implementation: Future. Business cases uncertain. Datataypes match DICOM Documentation more DICOMy. Documentation for FHIR users would be needed Implementation: Near future, near certain. Needed for VNA interfaces to viewers.

Imaging Object Selection DICOM KOS Manifest Format consistent with FHIR Better for communication of Study Metadata to EHR and EHR-based systems Implementation: future DICOM binary format No format conversion if to be used to retrieve instances Implementation: now, in XDS-I

Workflow FHIR Workflow UPS-RS / MWL / MPPS Potential to replace PACS-Driven Interpretation Workflow? Hazards of using PACS as report repository fro reading Additional EHR data useful UPS-RS Easier reporting integration MWL near-universal Supported by all modality devices SCP hosted by PACS MPPS Supported by many CT & MR Little used, little tested Potential for use in high throughput protocolling

Voice Rec / Reporting System RIS-Driven Workflow RIS EHR Copy of Report Orders Launches Display Report Launches MWL Query Modality PACS DSS Voice Rec / Reporting System Images Radiologist reads prior reports from RIS

Retrieve Instance FHIR WADO-RS FHIR may be important for retrieval of DICOM-encapsulated documents. EHR users do not expect to have to launch a DICOM viewer to view dicom-encapsulated documents. Is removal of DICOM “packaging” supported?
