Page No. 1 Name/Org: Nancy Self Phone #: ECR EO / PCN PB21712 Update Solo Planning Requirements for Increment 17 lDescription of Change: Update Increment 17 URC Planning Requirements for Solo. The Solo Increment 17 Planning Data Set is viewable via the PPS system under "Reports" by filtering on Increment "17" and Status "Working" - it is the file named "Solo Study Sess I17". Attachment is the Solo URC report as pdf for reviewer convenience. lJustification: Solo planning requirements need to be updated in the Increment 17 Planning Data Set. Late changes to requirements made significant changes to the baselined URC. lEffectivity: Increment 17 Description/Justification of ECR

Page No. 4 Name/Org: Nancy Self Phone #: ECR EO / PCN PB21712 Update Solo Planning Requirements for Increment 17 Resolved issue: The following comments were from HRF: Meganne Valerioti: “HRF recommends disapproval of the ECR unless ESA can verify that the changes contained in this ECR and any sessions under the jurisdiction of the ESA planners do not deviate from what was approved by CPHS/ HRMRB and the ESA Medical Board. Additionally, it should be verified that this URC does not deviate from the protocol pitched at the Informed Consent Briefing and for which the crewmember signed an Informed Consent Form. Any changes that affect crewtime and/or protocol will necessitate updated approvals from the abovementioned boards, a CEF to Annex 5 info, a delta informed consent briefing and a resignature on the Informed consent form.” Pasha Morshedi: “Concur with recommendation for disapproval in M. Valerioti comment below. A specific example of conflicting information: the informed consent briefing provided to the crew specifies 9 hours of inflight time, whereas the ECR to update SOLO URC specifies several totals, none of which are nine. Until we can assess the difference between what is in URC and what was briefed to the crew/approved by CPHS, we should not approve this.” Disposition: “Withdrawn based on CBD action being assigned. CBD action needs to be assigned to POIC to ensure coordination and completion of a meeting with ESA/SOLO, HRF, and POIC. This meeting should resolve any discrepancies between what was operationally planned and what was approved by the medical board (and briefed to the crew). Upon completion of this meeting and resolution of the aforementioned discrepancies, CBD action can be closed. Commenters concur.” The action should be assigned to Pat Patterson with a due date of June 30 th.
