Medicine For Elderly Services Part-time, 28.8 hours over 4 days Team of 4 therapists & 1 clinical support worker Receive referrals from wards at RVH & GPs across Dundee Patients seen in variety of settings: Assessment/ rehab wards Care home Clinic Patient’s home

Change Fund Support older people with changing health needs to continue to be cared for in a Care Home setting Programme 3Programme 5 At risk assessment & support Programme 7 Improve resources and support to people with dementia
