Intervention name Brief Description I. Run for the Rolls is a 1 mile fun run and a 5K race which will take place Saturday August 27 th In addition to the race, there will be a 10 week training program and a Couch to 5K training program to encourage people not just to prepare for the race, but to make walking and running part of their life in order to get adequate physical activity and maintain a healthy weight. This in turn will lead to improved health and improve and reduce chronic disease risk. II. Non runners and runners. Sedentary or moderately active to active. Families of all ages and abilities are welcome III. The 10 week training program C25K will start the second week of June, after school lets out. Training will end 1 week before race day, Aug. 22, Link to Coalition’s 1 & 5 year plans The training program, it provides guidance, motivation and support for families and individuals preparing for their first 1 mile race or first 5K. It also helps to connect with others gaining a running/ walking buddy or group. With the training program, School Challenge, Bulldog Challenge and the 1-miler 5K race, it offers community members a realistic goal incentives and affordable means to move more, eat better and to connect with others in a healthy way. St. Louis Center residents train for this event during the summer months. What health/wellness survey question(s) does this intervention address? It increases the portion of adults who report participating in physical activities or exercise (running, walking, yoga, gardening, calisthenics). Year 5: Chelsea Friends and Family Wellness Coalition

List all the goals of the intervention SMARTGoal 1: Specific Goal To follow the same program, but to increase family and 5k registrations. How success will be measured Not only by the number of participants but by the number of people who integrate it into their lives Describe what data will be collected and who will collect it Consent forms and follow up questioner of who participated in the training program. The registration forms will be the data for the race. Collected by the Race Director, Cindy Triveline Why do you think the goal is achievable? The 10 week and C25K Summer training program is a graduated pace for busy lives, people that cannot make every meeting will not loose out and can join right back in. Upon signing up for the Summer Program, all participant will receive both training programs if they are no able to attend. There are also printable document on the Run for the Rolls website if you do not want to attend the Summer training program and do it on your own or with friends. Is the timeline achievable Timeline is very achievable. The 10 week is consistent, not to drawn out. 1. To get community members of all ages and abilities to move more connect with others and learn to make healthier choices. Food, behavior. 2. Increase the number of registrations race day. 3. Impact families to train with us. 4. Learn how to make running or walking a Lifestyle. For the primary goal provide the following information: Year 5: Chelsea Friends and Family Wellness Coalition

Units of Engagement Estimated number of participants (300) Estimated time each participant spends at each event (1 hour) Estimated number of events to be held (1) 300x 1 x 1 = 300 Training program-10 participants x 45 minutes per training x 30 training days=225 Total Units=525 Key Evaluation Data A post survey is included in the handouts for the Summer training program. Number of participants. 243 – 2015 participants- 1 mile / 5K Number of people who complete the training program. 10 participants Summer Program. All participated in either the 1 miler of 5K. Number of repeat racers 80% of 1 mile race are repeats Year 5: Chelsea Friends and Family Wellness Coalition
