Victim Crushed to Death between Turning Forklift and NC Milling Machine Overview of Accident When: Sep. 22 (Fri.), 2007, 20:00 Where: In front of the NC milling machine within XX Industrial Company’s factory located in Gimhae, Gyeongnam Who: Victim, who was watching a forklift work How: The victim was hit by the back of the forklift (counterweight) as the forklift was turning right to carry an MCT. Why: The victim’s abdomen (stomach) was crushed between the forklift and NC milling machine. Ⅰ Ⅰ

Victim Crushed to Death between Turning Forklift and NC Milling Machine Details of Accident On Sep. 22 (Sat.), 2007, the victim began organizing things to prepare for a plant move with a co-worker. At 19:30, a truck loaded with an MCT arrived at the factory and the rented forklift driver unloaded the machine from the truck and put it app. 6m in front of the installation position within the factory. As the forklift unloaded the MCT from the truck on the machine’s side, the machine was turned upright and placed near the installation position. Ⅱ Ⅱ

Victim Crushed to Death between Turning Forklift and NC Milling Machine Details of Accident As the forklift drove forward to the gap between the MCT and NC milling machine (app. 6m) to lift the MCT from the front and turned right. While watching the forklift work, the back of the forklift (counterweight) hit the victim on the stomach and he was pinned against the NC milling machine. The victim was taken to a hospital but died during treatment. Ⅱ Ⅱ

Victim Crushed to Death between Turning Forklift and NC Milling Machine Details of Accident The minimum turning radius of the forklift is 4m and the position of entry where no contact with the NC milling machine is possible is at least 2.7m away from the A line shown on the accident situation diagram. However, the position where the victim was crushed indicates that the forklift did not reach at least 2.7m away from the normal entry point but made an abrupt turn to the right at the point 1~1.7m away from the entry point, pinning the victim to the NC milling machine with the tail part (counterweight), crushing him to death. Victim Crushed to Death between Turning Forklift and NC Milling Machine Cause of Accident Access allowed to areas with contact risks The forklift carried a machine while a worker was standing within the risk area where the forklift could contact the worker. Lack of preventive measures including forklift contact prevention There was no supervisor who could supervise and assist the forklift operations during the accident. Lack of skills in operation of the forklift The forklift operator should have considered the width of the entryway and minimum turning radius when turning to the side. The forklift turned at a position where the forklift could contact the victim and the risk became reality when it hit the victim with its back (counterweight), and crushed the victim to death. Ⅲ Ⅲ

Victim Crushed to Death between Turning Forklift and NC Milling Machine Prevention of Accident Prevention of forklift contact and safe operation When using a forklift, it is necessary to create an operation plan specifying the dimensions of the workshop, forklift type and capacity, load type and shape, operation path and method and perform operations as per the plan. A supervisor or a guide should be assigned to facilitate safe operation or driving in accordance with the operation plan. Also, the forklift operator should follow the guidance of the supervisor or guide and their signals. Ⅳ Ⅳ
