MA MFP Demo CMS Site Visit ■Centers for Medicare & Medicaid (CMS) will be conducting a 3-day Site Visit in July 2016 ■Purpose of site visit: –MFP Demonstration overview within Massachusetts –Discuss Successes and Challenges ■Dates are: –Monday, July 18 th (Administrative, Quality Improvement, Housing, Waivers; Discussion w/ Transition Coordinators, Case Managers and Service Coordinators) –Tuesday, July 19 th (MFP Participants Discussions, Interviews and/or Home Visits) –Wednesday, July 20 th ( Wrap Up Discussion) 9

MFP Home Accessibility Adaptations ■MRC arranges, plans for and oversees completion of MFP Home Accessibility Adaptations (HAA) ■Physical adaptations to participant residences necessary to ensure health and safety with independent function at home, such as; –Ramps, railings, stair glides –vertical platform lifts, overhead ceiling lifts –wider doorways, roll in showers, accessible kitchens –electrical upgrades, generators, flooring replacements ■Limited to $50,000 per participant ■MRC has recruited and contracted with 23 Architects and Construction companies 10

MFP Home Accessibility Adaptations A total of 171 HAA projects have been completed CY13 through CY16; ■Currently 53 Projects on-going; ■Average Cost per Project $34,000 11

811 Project Rental Assistance On-going Housing Opportunities There will continue to be opportunities, under the 811 program, for individuals transitioning from facilities, as well as for some waiver-eligible individuals the community 13

811 Project Rental Assistance Housing Resources 190 Project-based rental assistance units 25 Massachusetts Rental Vouchers 13 Housing Choice Vouchers (mobile) 12 Project-based Section 8 ■Units will continue to be identified until all subsidies are filled, likely through 2020 (properties identified in 2020 may not be occupied until 2022) ■Units are under contract for 30 years, which means eligible individuals will need to be identified and supported in filling these units upon turnover during this time period TOTAL UNITS

811 Project Rental Assistance Priority Categories In order to be eligible, the individual must meet low income requirements set by HUD and be under the age of 62 as well as meet one of these priority categories: 1. Enrolled in the Money Follows the Person Demonstration and currently in a long term care facility 2. Residing in a long term care facility and eligible for an HCBS waiver 3. Residing in a long term care facility and not eligible for either the MFP Demonstration or the HCBS waiver 4. Living in the community and eligible for an HCBS waiver 15

First 811 PRA Property: Georgetowne Homes in Hyde Park (Boston) Currently we have the following waiting list numbers: ■13 Priority 1 (MFP Demo eligible) ■6 Priority 4 (Waiver eligible in the community) ■Property will offer 4 One Bedroom units and 2 Two bedroom units ■Of this group, 2 require a 2 bedroom apartment; the remainder (17) are requesting a 1 bedroom. ■Please note that these units are small: 517 square feet for a 1 bedroom and 717 square feet for a 2 bedroom, which means that an applicant with large medical equipment may wish to apply for a 2 bedroom. 16

Housing Search Performance This chart shows a summary of the housing searches done by Housing Search Entities (HSEs), Transition Entities who are also qualified Housing Search Entities, and Transition Entities who did housing search without being a qualified Housing Search Entity. The chart summarizes the categories into two possible housing outcomes: people who find qualified housing, and people who did not find qualified housing. Money Follows the Person Demonstration “Wind-Down” 18

MFP Demonstration Wind-Down ■The Centers for Medicare and Medicaid Services recently notified all State Grantees they were reducing MFP Budgets for the remaining period of the grant. ■MA is expected to wind down approximately a year earlier than expected. ■Based on our current estimates, we anticipate ending transitions from facilities by 12/31/

Important Dates for MFP Wind-Down ■Aug 31 st, 2016: Last Day to enroll new enrollees into the MFP Demonstration ■Dec 31 st, 2016: Last day to transition and receive MFP Demo Services ■Dec 31 st, 2017: Last Day of service provision through the MFP Demo 20

Quality of Life Surveys ■Quality of Life Interviews (Surveys) will continue through the remaining calendar year of 2016 –Baseline (Initial) –11month –24 month ■Quality of Life surveys will no longer be required as of January 1,

MFP Demonstration Wind-Down MassHealth has recognized the importance of supporting the MA ADRC Partners in achieving our MFP goals. –Our goal was to use MFP grant to provide funding to ADRC Partners so that they would be able to build infrastructure, expertise, and institutional knowledge. –The MFP Demo has achieved great success: as of May 2016 we have transitioned 1,766 individuals to the community. –We have provided extensive training to the ADRCs. –We have worked together to address core barriers that impede successful community transitions. Specifically, this has included providing training on addressing housing barriers, as well as finding ways to generate more housing opportunities. –The availability of multiple HCBS waivers, and LTSS will continue to support individuals to transition back in the community. 22

MFP Demonstration Wind-Down –We want to especially note that the MFP Waivers will continue to operate and to accept transitioning individuals after the grant eventually ends. –We will add Transitional Assistance Services into these waivers to ensure that Transitional Assistance is available to members who participate in the MFP waivers. –We will also continue to have the HUD 811 Project Rental Assistance program available for individuals transitioning from facilities, and will: identify developers and housing for transitioning individuals provide up to 190 units of project-based housing as well as an additional 50 units through state provided vouchers. 23
