Present Situation

EMR, EHR and PHR Electronic Medical Record (EMR): An electronic record of health-related information on an individual that can be created, gathered, managed, and consulted by authorized clinicians and staff within one health care organization.

EMR, EHR and PHR (cont.) Electronic Health Record (EHR): An electronic record of health-related information on an individual that conforms to nationally recognized interoperability standards and that can be created, managed, and consulted by authorized clinicians and staff across more than one health care organization.

EMR, EHR and PHR (cont.) Personal Health Record (PHR): An electronic record of health-related information on an individual that conforms to nationally recognized interoperability standards and that can be drawn from multiple sources while being managed, shared, and controlled by the individual.

Outdoor Patient Management

Log in Screen….…This screen only for patient registration and admission counter. A relevant person need to enter an employee ID and a password to open the screen.

Registration screen. This screen contains some particular necessary data of a patient and a life time unique ID number.

Patient will also be given a smart card having with this ID number as well as some necessary information of the patient ………….

This is the patient’s out-door ticket or slip……  This out-door ticket contains a serial number with some necessary information and a bar-code sign.

This is a digital display board for patient’s queue management. This display board shows the patient name, ID, appointment time- type, serial number, arrival time, doctor’s name, department name,room number and floor number.

Log in Screen for Doctors. Doctor will be provided an employee ID and a password to open his/her dashboard.

This is a doctor’s dashboard.

At the bottom of the Clinical data template there are some other necessary fields like “ Additional Question, Previous Drug History and Side Effect of Previous Dose of Drug”

A completed clinical data sheet

This is a list of around 3500 ICD 10 Diagnosis code list provided by DG health.

This field is used for “Provisional Diagnosis” A doctor can write here independently.

This button bellow is used for giving lab investigation for a patient.

This is the special laboratory investigation list which are frequently used by the relevant department.

And this is the common laboratory list for every department.
