How Do I Login to Complete My Enrollment? Please visit and click Login : 3 Online Benefit Enrollment

What is My Username & Password? Username : the first six (6) characters of your last name followed by the first letter of your first name, followed by the last four (4) digits of your Social Security Number. Default Password: your full last name (lowercase, excluding punctuation) followed by the last four (4) digits of your Social Security Number. What’s New for : Benefit elections will become effective 9/1/2016 (elections requiring evidence of insurability, such as life Insurance, may have a later effective date, if approved). After annual enrollment closes, benefit changes can only be made if you experience a qualifying event (and changes must be made within 30 days of event). 7 Benefit Updates

What’s New for : Medical: Rates increased for the Scott & White HMO, ActiveCare Select & ActiveCare 2. Benefit Changes for all plans can be found on the Benefits Website. New Carriers! – Disability: Cigna Flexible Spending Accounts: If you currently participate in a Health Care or Dependent Care FSA, you MUST re-elect a new contribution amount every year to continue to participate. Cards are good for 3 years! – You are not eligible for HealthCare FSA if you are enrolled in a HSA. 8 Benefit Updates

Disability– Cigna Cigna is replacing The Hartford as the Disability provider. o If you are currently enrolled in The Hartford’s Disability plan you will automatically be enrolled to the corresponding Cigna Disability plan. Why Do I Need Disability? Disability is designed to provide a monthly income to an employee that is disabled due to an accident or illness. Elimination Periods are the # of days you must be disabled before benefits begin & vary from 0/7 to 180 days. – Employees electing an elimination period of 30 days or less, if you are confined to a hospital for 24+ hours due to a disability, the elimination period is waived, & benefits will be payable from the first day of disability. 2 Disability Options: – Premium or – Select All new or increases in coverage are subject to pre-existing condition exclusions. OneAmerica Voluntary Life & AD&D Birdville ISD offers $10,000 of Basic Life w/AD&D coverage to every eligible employee. Voluntary Life*: New Hires: You may elect up to $250,000 (cannot exceed 7 times annual salary) with no health questions required! Existing Employees: If you currently have coverage you may increase it by one $10,000 increment with no health questions (not to exceed the New Hire guarantee issue and/or 7 times annual salary). All increases above these amounts will require an evidence of insurability (EOI). Voluntary Spouse Life*: New Hires: You may elect up to $50,000 (cannot exceed EE’s amount) with no health questions required! Existing Employees: If you currently have coverage you may increase it by one $5,000 increment with no health questions (not to exceed the New Hire guarantee issue and/or EE’s amount). All increases above these amounts will require an evidence of insurability (EOI). Your FSA is administered by NBS (National Benefit Services). FSA’s allow you to pay for qualified expenses on a pre-tax basis, thereby reducing your taxable income. You determine the monthly contribution amount. Funds allocated to a FSA must be used during the plan year as they are Use-it-or-Lose-it! You must re-enroll in your FSA’s each year! Your contribution will not roll over! HealthCare FSA: – Pay for eligible medical expenses like deductibles, co-payments, prescription drugs, & other expenses not covered by TRS ActiveCare & dental or vision expenses. – Cards are good for 3 years! – You cannot enroll in a HSA if you have this plan. 21 Health Savings Account (HSA) Health Savings Account Plan (HSA) High Deductible Insurance Savings Account Protects you from big medical bills Helps pay your deductible Tax-deductible deposits Tax-deferred growth + A HSA is a unique tax-advantaged account you can use to pay for healthcare, dental and/or vision expenses. Funds contributed to your account never expire & can accumulate year after year. You must be enrolled in a High Deductible (HD) medical plan to qualify for a Health Savings Account (HSA) Maximums are $3,350 for individual & $6,750 for family. Ages 55+ have an additional “catch-up” contribution of $1,000. If you enroll in a HSA you are not eligible for the MEDLink Gap plan or a HealthCare Reimbursement (Flex).

What’s the Difference Between FSA & HSA? 22 FSA and HSA Comparison Participants with a Flexible Spending Account (FSA): 1. Funds are up fronted. 2. Funds do not roll year to year. 3. Can participate in a MEDLink (Gap) Plan. 4. Cannot have a HSA. 5. Can be enrolled in any medical plan. Participants with a Health Savings Account (HSA) : 1.Funds are NOT up fronted. You can only withdrawal what you have contributed to your account. 2. Funds will roll year to year. 3. Cannot have a MEDLink (Gap) plan. 4. Cannot have a FSA. 5. Must be enrolled in a High Deductible (HD) Plan.

There are special rules & requirements to receive the pre-tax benefit election plan privileges: Birdville ISD must set a plan year. The plan year is: – September 1 – August 31 of each year. Although coverage is voluntary, every employee is required to review their current elections & make changes as desired by completing the Birdville ISD online enrollment. Any pre-tax elections will remain in effect unless you have a qualified event change in family status. Qualifying event benefit changes must be made within 30 days of the event & changes must be consistent with the event. Qualifying Event Examples: – Marriage, Divorce, Birth or Adoption, Death, Change in Dependent Eligibility Status, etc. 24 Section 125 Cafeteria Plan Amendments
