The Road Ahead Healthcare Forum Thursday 3 rd March 2016

The story so far…. A journey of adventure with lots of changes along the way:  A Health Authority  A Primary Care Trust  A Clinical Commissioning Group

The story so far…. NHS Five Year Forward View (5YFV) published 2014 and set the direction of travel nationally setting challenges and opportunities

The story so far…. Dudley CCG selected to become a ‘vanguard’ after competitive process – we can try new ways of working with support and this can be rolled out across other areas. This is called new model of care – changing the way we do things

The story so far…. Our vanguard is a ‘multi specialty community provider’ (MCP) which means that we are joining up health and social care and putting the patient at the centre of their care

The story so far…. Our MCP is called All Together Better – it is a partnership of Dudley CCG, Dudley Group of Hospitals NHS Foundation Trust, Dudley & Walsall Mental Health Partnership Trust, Dudley Council, Black Country Partnership Foundation Trust & Dudley Council for Voluntary Services

The story so far….

What is the Operating Plan? It sets out the things that we will do to improve health outcomes and the quality of health and care services for our population within the resources allocated to us. It will now also be ‘year 1’ of a 5 year ‘Sustainability & Transformation Plan’

What next? It’s still business as usual and we need to look at the priorities for the year ahead

Operating Plan Influenced by:  The needs of our local registered population  Clinical priorities  National ‘must dos’  What patients tell us is important

What’s in Dudley’s Operating Plan  Targets for how we will address local health inequalities through preventative work on obesity, smoking and alcohol consumption  How we will develop and commission the Multi-Specialty Community Provider and deliver more integrated health and social care, particularly for the frail elderly, those at the end of life and patients with long term conditions  How we will commission “best practice pathways” for planned care and meet required waiting time standards  Improving our urgent care pathways for physical and mental health and commissioning services to prevent unnecessary admissions  Commissioning primary care using a new contractual framework  Improving services for people with mental health problems, including children and young people  Ensuring people with learning disabilities can be looked after in the community, avoiding the need for admission to hospital

Areas of challenge  Childhood obesity  Dementia  Increase in women smoking

Sustainability & transformation plan (STP) We need to work with neighbouring CCGs People in Dudley use hospital services in neighbouring areas (particularly Wolverhampton and Sandwell) How do we ensure everyone receives the same standard of care, regardless of where they go? If neighbouring services fail it affects our local services (e.g. Staffordshire impact on Wolverhampton) Hospital care needs to reform and become more efficient (so we can afford more community-based care)

Finance – 2015/2016 position

So what will Dudley CCG do?  Implement New Model of Care  Shift resource: community where possible and hospital where necessary  Work with Dudley Group of Hospitals NHS Foundation Trust to make hospital care as efficient as it can be  Reduce ‘variation’ in all healthcare procedures  Promote people to self-care with support
