Basic Professional Training Course; Module X Operational safety including operational feedback Introduction Students from different countries (which they represent) should be divided into groups – presuming it is an international course. If it is a national course, students should be divided into groups. If possible, regulators, operators and representatives from the industry should be in mixed groups to make the maximize exchange of information between them. Also it is a learning opportunity for them to have a different perspectives. All the groups will participate in addressing all the topics and prepare a short presentation for the plenary session. 2

An OSART (Operational Safety Review Team) mission result good practice is an outstanding and proven performance, programme, activity or equipment in use that contributes directly or indirectly to operational safety and sustained good performance. 3

Good practice: The plant personnel and contractors are actively involved in safety improvement process. Task: − Establish a process in which anyone can suggest findings on the site for improvements! − How to evaluate and in timely implement proposals? − How and where to display results of the evaluation or implementation of the improvements? − How to further promote the improvement process and encourage personnel to participate? 5

Good practice: The Plant has used an effective process to develop and implement a new Web Based Training Package for its Quality Management Program. Task: How to involve plant staff in the selection of the topics and in the development of the contents? What should be included in the training? 6

Good practice: Timely capture of human factor and safety culture data during refuelling outages. Task: How should the plant collect event related data? How to compose a team of experts (from which departments)? For which purposes the data could be used? When the on-site investigation should be commenced? 7

Basic Professional Training Course; Module X Operational safety including operational feedback Case studies Evaluation Each group should discuss and present their findings in a final plenary that can last 1 – 2 hours depending on the time available – discussion in the plenary by all participants. Lecturer should present the good practices (on the following slides) and make comments and suggestions to each working group presentation. 8

Good practice: The plant personnel and contractors are actively involved in safety improvement process. Kashiwazaki 3/6, Japan, OSART mission 1-18 Nov The plant has established a process in which anyone can suggest findings on the site for improvements. Proposals are regularly evaluated and in reasonable cases timely implemented. The bulletin boards at the entrances to the plant units are used to display results of the evaluation or implementation of the improvements. These boards further promote the improvement process and encourage personnel to participate. 9

Good practice: The Plant has used an effective process to develop and implement a new Web Based Training Package for its Quality Management Program. Mühleberg, Switzerland, OSART mission 6-23 Nov The Plant has used an effective process to develop and implement a new Web Based Training (WBT) Package for its Quality Management Program. The project team obtained senior management support for the initiative and then involved plant staff in the selection of the topics and in the development of the contents. This development technique significantly strengthened the outcome of the training package. 10

The plant also took advantage of lessons learned from previous WBT projects and produced a training package that provides good training for the subject. The training has been very well received by those who have taken it. Of particular note is the integration of plant staff in the supporting photos and the use of a practical example of a pump replacement to illustrate the Quality Management process. The result is a package that the plant can be proud of, and a process that will serve it well as the basis for future WBT modules. 11

Good practice: Timely capture of human factor and safety culture data during refuelling outages. Oskarshamn, Sweden, OSART mission 16 Feb-5 Mar The plant has established an on-call team that it is constituted during outages to collect event related data. This team is composed of thirteen skilled and experienced personnel, including human factors and safety culture experts, and can be gathered promptly in cases where a significant event could occur in the outage. 12

The data will be used for subsequent a root cause analysis for events having organizational or safety culture related contributing factors. As stated in the IAEA standards, the on-site investigation should be commenced as soon as practicable after the event occurrence, in order to ensure that information is not lost or diminished, interviewed personnel have a clear notion of the sequence of events and evidence is not removed. This on-call investigation team allows the plant to achieve these goals. 13 The views expressed in this document do not necessarily reflect the views of the European Commission.
