A medical home is the ONE place you and your family can go for ALL healthcare needs. Patient Centered is a way of saying that you the patient, are the most important person in the Health Care System. ABCCEDP The Alabama Breast and Cervical Cancer Early Detection Program (ABCCEDP) provides free breast and cervical cancer screening for women ages 40 to 64 who meet income eligibility guidelines and are uninsured or underinsured. Services include a pelvic exam, pap smear, clinical breast exam, mammogram and diagnostic services, such as an ultrasound, colposcopy or biopsy, if needed. For more information, call Alabama Wisewoman Program This program provides cardiovascular risk screenings and health education on diabetes, cholesterol and blood pressure for participants of the ABCCEDP. One-on-one nutritionist consultation and monthly support group meetings are included with participation. For more information, call Medical Social Work Trained social workers, counselors and therapists are a part of the Social Services Department. Patients are eligible to receive counseling services for many concerns including mental health, substance abuse, stress management, crisis intervention and referral services. For more information, call Case Management Case managers assist with coordination of health care by collaborating with the patient’s primary provider in assessing, planning, implementing, monitoring and evaluating services to meet patient needs. Case managers also assist with safe transitioning between health care levels. This unique machine is portable, which allows the clinic to bring mammography services to patients in several of site locations. A referral is needed from a medical doctor, physician assistant, chiropractor or nurse practitioner. For more information, call Occupational Health Center This service is offered through the Gulf Region Health Outreach Program (GRHOP), allowing patients with chronic respiratory, asthma problems, work related musculoskeletal pain or other work related issues to be referred to the Occupational Health Center with the possibility of receiving diagnostic testing.
