Confidentiality & HIPAA Indiana Family and Social Services Administration (FSSA) requires a properly completed Authorization for Release of Case Status Information form be on file prior to the release of any account related information.

Authorization for Release of Case Status Information A Client Release form should be voluntarily completed by applicant parent or guardian. One Client Release must be properly completed and on file for each enrollee (child) prior to disclosure. Make sure to initial where indicated. Signature is required. The Client Release expires after 1 year.
