About the ALLIANCE The Long Term Conditions Alliance Scotland (LTCAS) formed in 2006 to advocate for a shift to asset-based approaches, articulated through the self management agenda. LTCAS became a powerful alliance bringing together a wide membership and building an influential position as strategic partner, and critical friend, of the Scottish Government. Board feels strongly that we now need to look to the future and adapt to the changing policy context. Central to this has been re-naming the organisation.

About the ALLIANCE The ALLIANCE’s vision is for a Scotland where people of all ages who are disabled or living with long term conditions, and unpaid carers, have a strong voice and enjoy their right to live well, as equal and active citizens, free from discrimination, with support and services that put them at the centre. Over 270 members; mainly third sector organisations, along with statutory and private sector associate members and individuals who use health and social care services. Membership spans long term conditions, mental health, disability and unpaid carers.

Strategic partner of Scottish Government. Manages £2m a year Self Management Fund for Scotland. History of partnership, innovative programme development/delivery, with voice of people at its heart. Provides a voice for users of health and social care services at a national policy level. Works as a conduit between people and decision makers. What we do

Change Fund: Enhancing the Role of the Third Sector- programme designed to boost influence and impact of the sector within Reshaping Care for Older People. ‘My Condition, My Terms, My Life’ self management awareness raising campaign launched First phase focused on raising public awareness and now on health and social care professionals. Self-Directed Support and Health: Building the Connections. Looking at the question of how self directed support and health will interact under new legislation and forthcoming integration. A wide range of projects: just a few examples

‘My Skills, My Strengths, My Right to Work.’ Project focusing on gathering information and experiences around employability, inequality and successful self mangement in employment. Improving Links to Primary Care. Partnership with RCGP making the case for General Practice developing links developing links with community resources. Walking towards better health. Partnership with Paths for All and this Project has collected information about the barriers and enablers to physical activity people with long term conditions. National Person-centred Health and Care Programme- ALLIANCE was invited to lead co-production strand. People Powered Health and Wellbeing Programme Director now in post. Programme Director will drive forward work to help change culture across sectors. ALLIANCE Involvement Network and Dementia Carer Voices Project - part of a wider programme drawing on voice and personal experience. Dementia Carers Voices project. New project building on the awareness activities from 'Tommy on Tour ' project and the ALLIANCE. Listening to people’s voices
