Log into your Google Account:

Click on the array and select Drive.

Click on Create and select Form.

Type in a title for the Parent Contact Info form and choose a theme.

Type a question to include on the Parent Contact Info Form.

There are different question types.

Check the box if the question must be answered.

Add more questions as needed. (refer to document: XX Parent Contact Form XX)

Decide choices for the confirmation page.

Click on “view live form” to see parent’s view of form.
