He had experienced 2 syncopal episodes in conjunction with auditory stimuli, and collapsed shortly after an alarm clock sounded and was resuscitated from ventricular fibrillation. The resting QT is unremarkable, but prolonged by 60 ms with 0.10 μg/kg/min adrenaline, and failed to shorten with exercise. The ambulance 12-lead and day 1 post-arrest ECG show J-wave elevation that is initially slurred and subsequently notched. Genetic testing for LQTS genes was negative, and there were no family members with similar findings.
