1. New License Application Submit an online application Application under review Communication for additional requirements and/or EXAM Register for Exam (if applicable) on Prometric.com Submit requested information Primary Source Verification process (if applicable) Decision and communication Submit further requirements (MMI, AMML, BLS) License issuance AED 1020 Fee as per category AED 520 MMI: Medical Malpractice Insurance AMML: Al Maktoum Medical Library BLS: Basic Life Support Applicant PLD Fee applies

2. License Renewal Must be submitted 30 days prior to expiration date Not applicable to: expired license, LOA, Inactive license, Visiting license. Incomplete application may be rejected Unpaid application shall not be accepted Expired license will apply according to “ Late Renewal Policy” Applicant PLD Complete the Renewal Application Form Complete the Professional Development Portfolio Submit to DHCR office with payment receipt Review and processing Decision & communication License Issuance Fee as per category

3. License Renewal- Late Renewal Applicable to any application after expiration up to 6 months. More than 6 months will be required to Reapply Penalty fees applies according to duration (please refer to the fee structure) If license expired being INACTIVE, applicant must reapply Applicant PLD Complete the Renewal Application Form Complete the Professional Development Portfolio Submit to DHCR office with payment receipt Review and processing Decision & communication License Issuance Fee as per Category + Penalty + fee per each month of delay

4. Upgrade a License Applicable to: Specialist  Consultant General  Specialist Assistant  Full Must be licensed in DHCC prior to submitting an upgrade request Must be on Good Standing and no cases are pending Applicant PLD Submit upgrade request via Assess and communicate eligibility requirements Submit requirements and fees Process, PSV, Decision Communication and request licensure requirements Submit requirements (if applicable) License issuance Fee as per renewal

5. Add a Title Only applicable for Medical / Dental specialists Must show evidence of full qualification for the additional title Fee are applicable as per renewal structure New title and New license number will be issued Professional can hold a maximum of 3 Active titles at a time Applicant PLD Submit a request via Assess and communicate eligibility requirements Submit requirements and Fees Process, PSV, Decision Communicatio n and request licensure requirements Submit requirements (if applicable) License issuance Fee as per renewal

6. Add employer (Part-Time) Only applicable to: General Medicine/Dentist Medical Specialist/ Dental specialists A physician can be listed under a maximum of 3 facilities at a time The specialty of the applicant MUST be on the Clinical Operating Permit of all applicable facilities Supporting Document: No Objection Certificate from Primary DHCC employer Job offer from new facility MMI to cover the applicants in all applicable facilities New passport size Photo for license issuance Applicant PLD Complete a Change of Status Form Submit documents + receipt via Review and process Communicate confirmation Issue License AED 2020

7. Change Employer Applies for all licensed categories This service is required: When leaving a DHCC facility to join another When activating a license under a new facility Supporting Documents: End of Service letter from current DHCC employer Job offer from new employer MMI to cover the applicants in all applicable facilities New passport size Photo for license issuance Applicant PLD Complete a Change of Status Form Submit documents + receipt via Review and process Communicate confirmation Issue License AED 2020

8. Request a Letter Available letters: Good Standing Certificate: Regular – AED 520 Urgent – AED 1020 Primary Source Verification ‘PSV’ – AED 520 License Status Confirmation ‘LSC’ (for Physicians & Dentists only) – AED 1020 Processing time 10 working days post documents & fees submission. 2 working days for Urgent ‘CGS’ requests Duplicate, change in details, and change license status will require second payment Applicant PLD Complete the request letters form Submit documents + receipt via Review documents and prepare letter Communicate readiness for collection Fee as below

9. Reactivation of License Applicant PLD Only valid licenses can be Activated, if the license is expired, Kindly reapply as per New License process 5 working days to complete post submittal of all required documents and fees Fees as follows: Activating with same employer after 60 days from Deactivation= AED License Card Replacement Activating with new employer= AED 2020 Required Documents: Change of status form Receipt of fee if applicable Job offer from a DHCC facility MMI to cover applicant under the new facility Passport Size photo for license issuance Evidence of activities/employment during the deactivated period Complete the Change of Status form Submit requirements + receipt via Review documents and process Communicate completion of process Issue License Fees apply

10. License Card Duplicate/ Replacement Each second license card= AED 220 Requires a new passport size photo with white background Applicant PLD Send a request + picture + fee receipt via Process and Print license Communicate readiness for collection AED 220
