BOOSTER CLUB GUIDELINES MIAMI PALMETTO SENIOR HIGH ACTIVITIES AND ATHLETICS

CHECKLIST: WHAT ARE YOU MISSING?  Constitution/ Bylaws  Roster of Officers and Sponsor/ Coach: Due by first board meeting or fundraiser  Roster of Members: Due after first general meeting  Official Mailing Address: Due by first board meeting or fundraiser  Copy of Tax Exempt: Apply for one, you can’t use the schools!  Must have an EIN and need to file IRS form 1023, Application for Recognition of Exemption.

CHECKLIST: CONTINUED  Copy of General Meeting Minutes: Due after general meeting  Copy of Articles of Incorporation:  Create a Florida Nonprofit Corporation on Sun Biz  Apply to be a 501c3 tax exemption with IRS

LIABILITY INSURANCE  You MUST have Liability Insurance to hold fundraisers and Booster Events  Protect yourself, the school, the school board  MUST include the School Board of Miami Dade County as additional insured on the policy  Tell the Insurance Agent that this is a Booster Club policy, they should cost around $175 for a $1,000,000 policy

BOOSTER HANDOUTS/ REQUESTS  Remember you CANNOT require payments or donations from families. Any money given by individuals or businesses is considered a donation and must be self initiated. DO NOT solicit funds unless you have received approval from the school/ and or region  Fundraising form 6571  All handouts given to parents or students having to do with booster fundraisers, events, or donations must go through the activities and/ or athletics office accordingly.  Do NOT send anything out with prior approval from these offices.

FUNDRAISING FORMS  To request fundraising events, you must fill out the following forms  On Campus Fundraisers: MDCPS Form 6570 (Signed by the principal)  Off Campus Fundraisers: MDCPS Form 6571(Signed by the Principal and the Region) All forms must be typed! Forms located on school site:  Palmetto Site- Activities Palmetto Site- Activities

CONTRACTED SERVICES AND INSTRUCTORS  Any contracted instructor must be a level 2 volunteer or a Dade County vendor, submit a contracted services (form 2453) form, & receive payment with a school check.  BOOSTERS CANNOT PAY INSTRUCTORS!  Donate funds to pay them and then have sponsor pay them with a school check.
