 Registration needs to be done immediately.  Register at your home school  Review enrollment process Review enrollment process

 Birth Certificate OR other reliable proof of identity and age, including the student's baptismal certificate, an application for a social security number, or original school registration records.  Immunization Records OR proof of immunization exemption. PUHSD school nurses can provide FREE immunizations if needed.  Documents that are helpful, but are not required are: Current Transcript, State Test Scores, Transferring Grades and Withdrawal Form, Special Education Records (if applicable), and Guardianship Papers (if applicable)  If you have questions about enrollment, please call the school registration office.
