Clopidogrel (red dots), after conversion to its active metabolite (green triangles), antagonizes P2Y12, the platelet ADP receptor. In diabetes mellitus (DM) patients treated with clopidogrel, impaired platelet P2Y12 blockade is largely attributed to marked reductions ( ∼ 40% lower than nondiabetic subjects) in the pharmacokinetic profile of clopidogrel’s active metabolite (drug exposure) and attributed to a much lesser degree to altered functional status of the P2Y12 signaling pathway (drug response).
