B:7 - Living Well in Communities: There’s No Place Like Home

Susanne Miller Susanne Miller has worked in residential care for older people and as a child and family social worker in Glasgow. She worked in strategic planning for almost 10 years before returning to operational practice as a Senior Manager within children's services. In 2012, Susanne became the Assistant Director of Social Work Services in Glasgow, moving in 2015 to become Chief Officer Strategy, Planning and Commissioning and Chief Social Work Officer for Glasgow City Health and Social Care Partnership.

June Wylie June has spent over 20 years working as an Occupational Therapist and manager in a range of acute services, including acute medicine, medicine of the elderly and stroke in Glasgow, Ayrshire and Edinburgh. For the last twelve years June has worked in national posts at Healthcare Improvement Scotland and the Scottish Government. June was appointed to the post of Head of Implementation and Improvement at Healthcare Improvement Scotland in Her remit currently includes Living Well in Communities, Older People in Acute Care and Focus on Dementia. Living Well in Communities Living Well in Communities is a portfolio of improvement programmes, which aims to support people to spend more time living well at home or in a homely setting. We are supporting a selection of Health and Social Care Partnerships with improvement work, and we will be sharing the learning from these tests of change across all partnerships.

This is a clinically-led health and social care programme that extends across hospital, community and individual settings to deliver the overarching outcome of no unnecessary hospital stay and no delay in care intervention.

Palliative and end of life care Caroline Sime is a Research Fellow at the University of the West of Scotland and Ardgowan Hospice. The priorities for the project are to improve the waiting times from referral to services starting, improving patient experiences and introducing the use of patient reported outcome measures in a variety of hospice services. Living Well in Communities By providing targeted improvement support in these different care and service areas, and sharing our learning across Scotland, Living Well in Communities will enable more people to spend more time living well at home or in a homely setting.
