> PDE Office for Safe Schools Climate Surveys Open a web browser and got to the following URL: If you are taking the survey on school grounds in a classroom or proctored setting and have been provided a username and password, please skip to slide 9 as the following are directions for respondents who have been provided an invite code only.slide 9 2

> PDE Office for Safe Schools Climate Surveys This section is for those survey respondents who have been provided a code in order to take the survey. Please click the link as indicated in the screenshot below to enter the provided invite code: 3

> PDE Office for Safe Schools Climate Surveys 4 You will be prompted to enter the provided invite code and then click the button named ‘Apply New Code’

> PDE Office for Safe Schools Climate Surveys 5 Once the code has been validated to not have been used on the computer previously, you will be directed to a survey for entry. You will notice that each question has a more information icon the you can simply mouse over to further help understand what is being requested of you for the question. There is also a Spanish version of the survey available by clicking the link at the top of each survey within the title bubble section which is illustrated within the following slide.

> PDE Office for Safe Schools Climate Surveys 7 Upon completion of the survey, click the ‘Save’ button at the bottom of the form. If there are any issue to one or more questions you will be presented with a screen similar to the image as shown below. Please address the listed issues and click the save button once again.

> PDE Office for Safe Schools Climate Surveys 8 A successful completion of the survey will direct you to a page similar to the image below. Thank you for completing the survey and your continued support. This is the end of instruction for this section and the start of those taking surveys by classroom or proctored setting.

> PDE Office for Safe Schools Climate Surveys 9 This section is for those survey respondents who have been provided a classroom username and password to take the survey. Please select the login link at the top right corner of the screen, as illustrated below. You will then need to enter your username and password into the text boxes and click the button ‘Log In’.

> PDE Office for Safe Schools Climate Surveys 10 After successful logon you will be presented with a screen similar to the image below. Please select the menu item, as circled in red below, named ‘Climate Survey’.

> PDE Office for Safe Schools Climate Surveys 11 You will now be directed to the survey selection screen, please select the applicable survey from the listed provided. You can do so by clicking the corresponding survey’s line item button as illustrated below:

> PDE Office for Safe Schools Climate Surveys 12 Upon clicking the Open survey link from the previous slide, you will be directed to a survey for entry similar to the one as shown within the following slide. You will notice that each question has a more information icon the you can simply mouse over to further help understand what is being requested of you for the question.
