PRACTICE PROFILE Heaton Moor Medical Centre is a combination of three practices with a combined patient list of 30,000 patients - Heaton Moor Medical Centre, Dean Lane Surgery in Hazel Grove and Little Moor Surgery in Offerton. Our Doctors and large support teams offer a varied range of services to our patients. Heaton Moor Medical Centre opened in 1971 taking in just six Doctors originally. The practice was totally renovated and extended in 1994 and extended again in 2002, 2009 and more recently in We are an innovative Practice and we are always looking for ways to improve our services for patients, through participation in pilots for new ways of providing care, as well as adapting to feedback and suggestions from patients. A TRAINING PRACTICE The Practice participates in the advanced training of fully qualified Doctors who have chosen to specialise in General Practice. The additional Doctors (Doctor Registrars) work at the Practice for up to one year and share all aspects of medical care with the Partners and salaried Doctors. During their time here, we hope you will view them as we do, an integral part of our team. We also occasionally have student Doctors and nurses attached to the Practice. You will be advised accordingly, and if you wish to see the Doctor or nurse alone, your wishes will be respected.

HOME VISITS In order to provide a good service to all our patients we request that home visits are only requested by patients who are too ill to come to the surgery. Routine Home Visits are usually in the afternoon, after Doctors have finished morning surgery. To request a Home Visit please call the Surgery on (0161) , before 10:00am, Monday to Friday, except in an emergency. HOW TO REGISTER If you wish to register with the Practice please attend the Practice and ask the Receptionist for a registration form. They can advise of any documentation you may need to register. SHARING OF PERSONAL INFORMATION Personal information about patients will be shared within the healthcare team, including administrative staff and other staff who support the provision of their care. COMPLAINTS In the unlikely event that you do not feel completely satisfied with your health care provided by the surgery, we would ask that you telephone the surgery to speak to the Practice Manager or Supervisor who will be able to assist you in your concern. If you wish to make a formal complaint then you should write to the Practice Manager detailing your complaint. ABUSIVE BEHAVIOUR We have a zero tolerance policy to violent or abusive behaviour within the Practice.

SUMMARY CARE RECORD What is the Summary Care Record? The NHS in England is using an electronic record called the Summary Care Record to support patient care. All the settings where you receive healthcare keep their own medical records about you. These places can often only share information from your records by letter, fax or phone. At times, this can delay information sharing and this can affect decision making and slow down treatment. About your Summary Care Record Your Summary Care Record contains important information about any medicines you are taking, any allergies you suffer from and any bad reactions to medicines that you have previously experienced. Allowing authorised healthcare professionals to have access to this information will improve decision making by doctors and other healthcare professionals and has prevented mistakes being made when patients are being cared for in different care settings. Your Summary Care Record also includes your name, address, date of birth and your unique NHS Number to help identify you correctly. Every patient is automatically opted into Summary Care Record unless they have opted out with their previous GP Surgery. However, when registering with the Practice, you can choose to opt out by asking for an OPT-OUT form at Reception and filling in the form for each person in your family. ACCESSIBLE INFORMATION STANDARD The Practice normally communicates with patients via letter for routine needs and telephone calls for urgent matters. The Accessible Information Standard applies to patients who have information or communication needs relating to a disability, impairment or sensory loss. It also applies to parents and carers of patients who have such information or communication needs, where appropriate. Individuals most likely to be affected by the Standard include people who are blind or deaf, who have some hearing or visual loss, people who are deafblind and people with a learning disability. If you need help communicating with us, please let us know and we can discuss your needs. For example: We can send your letters in large print. We have an induction loop system fitted. We offer online appointment booking. This list is not exhaustive, please discuss your needs with us. VIEWING YOUR MEDICAL RECORDS ONLINE The Surgery has a clinical system (EMIS Web) that, through a website, allows patients to easily and quickly view their electronic medical information held about them by their GP Practice, order repeat medication and book appointments online. You can now use the internet to securely view your appointments, test results, immunisations and your medical history, including current and past medication. If you wish to view your Medical Records online, you will need to ask at Reception for information on this. Your information will not be made available without your permission. If you decide not to join, or wish to withdraw, it will not affect your treatment in any way. *Once patients reach the age of 12 they are generally regarded as being "Gillick Competent" - meaning that they are able to make their own informed decisions about healthcare (sometimes with adult support). At this stage we are obliged to remove parental "proxy" online access and ask the young person to re-register themselves - this is in some circumstances, to protect their privacy if they wish to do so.

Practice Roles Nurse Practitioners – The Nurse Practitioner’s role is seeing patients with minor illness or injury as referred by Doctors. Duties Include: Giving advice on diet, exercise and smoking – potentially including prescribing nicotine replacement therapy if appropriate, provision of family planning advice including the prescribing of oral contraceptives, injectable contraceptives, diaphragms and emergency contraception Practice Nurse – Duties include but are not limited to: Child Health & Immunisations, Travel Immunisations, Cervical Cytology, Women’s Health, Asthma & Diabetes, liaise with Doctors, Health Visitors, District Nurses, Health Care Assistants District Nurses – The District Nurse team provides a wide range of nursing services to those who are housebound and to people whose care can not be delivered in a more appropriate place. Their services range from simple tasks such as injections, eye drops and venepuncture to complex wound care and terminal care packages Treatment Room Nurse – The Treatment Room Nurses provide services daily on each weekday. The Treatment Room caters for those patients who are able to attend the centre for their care and includes many of the same services mentioned under the District Nursing role description. To book an appointment with the Treatment Nurse please telephone who book appointments for all the Practices in the Stockport area Assistant Practitioners – The above plus clinics on chronic disease management Health Care Assistants – Main duties include: Phlebotomy (Bloods), New Patient Checks, Performing ECGs, Health Screening, Smoking cessation as required by Doctors Phlebotomist – The taking of bloods as requested by the Doctors Diabetic Nurse Specialist – The management of diabetic patient care. In the absence of a specialist nurse, this role will be covered by Doctors and Practice Nurses Health Visitors – The Health Visitors cover the Health and Development Surveillance of 0 – 5 year olds on Doctor’s Practice Patients Administrative Staff – Our team of receptionists, secretaries, general administrative will be your link with the medical staff at the surgery Management – The main duties of the Practice Manager include: Ensuring delivery and provision of services are met; Dealing with all non-clinical aspects of the practice’s organisation MANAGER Mr Ian Stanyer CLINICAL NURSING TEAM Nurse Practitioners Practice Nurses Assistant Practitioners Health Care Assistants Phlebotomists

PRACTICE FACILITIES GENERAL FACILITIES Treatment Room; Physiotherapy Room; 31 Consulting Rooms, male and female toilets, waiting room, Clinic Clerk, Baby Room and a Baby Changing facility in the Disabled Toilets DISABLED FACILITIES Patients will find the design of the improved premises includes two wheelchair lifts, 2 disabled parking spaces, and a disabled toilet easily accessible from the waiting room Please ask for further assistance if necessary As there are only two disabled parking spaces – use is limited to disabled patients only TOILETS Male and Female toilets are opposite Reception and a disabled toilet is based near the Practice entrance on the ground floor and also in the blue waiting area ON-SITE PHARMACY There is a private Pharmacy situated in the same building as the Practice (Well Pharmacy) – it is not owned or managed by Heaton Moor Medical Group. It offers the same services as any other Pharmacy as well as being very convenient for prescription collection PRAM STORE There is a covered Pram Store located next to Reception RESULTS When you see a member of the primary care team at the surgery you may have bloods, urine and other tests. Before you leave you will be given an indication of when to contact the surgery for the results of these tests Please telephone for these results after 2:00pm Please note that the Receptionists cannot give out results unless a Doctor has seen the results first
