How to Create a New USNA Form using the Official Design Standards

Open Adobe LiveCycle Designer ES4 (search your computer if it isn’t in the startup menu) Select New Form (New from Template if there is already a template designed using the design standards) Select use blank form (importing PDFs usually causes more re-work) Page size should be default (8.5 x 11) unless you are creating a postcard or it is a special circumstance where a different size is needed Select your orientation. Most forms are done in portrait style Select number of pages (this can be edited later using the Object palette) Opening the Software

Repeat this process for other text boxes you would need for the form 6. Templates allow you to save settings that are common to all official forms such as the border box and USNA in the form number 1. Click on Master Page 2. Ensure Object boundaries is on by selecting the View Menu-Objects-Object Boundaries 3. Create a border box (directions previously discussed) 4. Create a text box for form #, RCS or OMB, and supporting directive (directions previously discussed) Creating a Template

Select File Menu-Save. 6.Change the Save as Type to “Adobe LiveCycle DesignerTemplate (*.tds)” 7.Choose your location and filename and save 8.Template Manager from the Tools Menu and click the small arrow at the top right of the screen 9.Select Add Category and enter the name of the category that will store the form template (the added category should appear as a tab on the top) Creating a Template

10.Right click in the white space to get the Context Menu and select Add Template 11.Locate the.tds template file you created and saved on your computer and click open 12.The Template now appears in the white area 13. Close out of the Template Manager Creating a Template

Forms are designed in box style with fillable fields having upper left captions. Fillable text is 10 pt Times New Roman 1.Select your “(untitled Subform)(page 1).” Ensure you are not on the Master Page (this will make sure you do not move the border box or standard text boxes) 2.Select Text Field from the Standard palette of the Object Library and drag it onto the page 3.With the text field selected, change the borders to solid.0069in in the Border palette 4.With the text field selected, change the Caption to Top from the Layout palette 5.With the Text Field highlighted, choose Font from the Window Menu 6.Select the small arrow to the top right and choose to edit the font of the caption or the value (never edit together because the fonts are required to be different styles and sizes) 7.Highlight the Text Field and drag it to the correct place on your form Adding Fillable Text Fields

Open your form in AdobePro and select Save As-Other-Reader Extended-Enable More Tools-(form fill and save) and hit Save Now Saving a Form as Reader Extended
