West Gables Rehabilitation Hospital 2015 Stakeholder Report: Outpatient Stroke Program For more than 25 years, West Gables Rehabilitation Hospital has made a mission of helping patients reach their functional goals, while providing the highest level of post-acute medical care in a caring and culturally sensitive environment. West Gables is devoted to the rehabilitation of patients with various disorders and conditions such as; Stroke, Brain Injury, Spinal Cord Injury, Amputation, Neurological disorders, Orthopedic and Musculoskeletal conditions. Additionally, our specialized rehabilitation programs are designed to meet the needs of individuals who have a broad range of acute and/or chronic illness that cause temporary or permanent disability. Our Comprehensive Outpatient Stroke program features an interdisciplinary team of physicians, therapists and neuropsychologists who are specifically trained and certified to help patients and families recover after stroke with the overall goal of challenging a normal return to life. The Stroke program provides a thorough educational series for patients and families and utilizes internal and community resources to address all of the patients needs during his or her stay. An open access Q&A session with the program’s medical director as well as a comprehensive neuropsychology program are available to our patients and families. Our therapy team utilizes state of art equipment such as the Bioness Vector, Lite Gait, Vital Stim Experia and Bioness L300 and H200 to facilitate recovery in rehab. Our Outpatient rehabilitation program offers patients an individualized plan of care for physical, occupational and speech therapy and neuropsychology services. Frequency and duration of the plan of care is based on individual needs and determined at time of the initial evaluation. Typical treatment sessions are minutes in length, 2-3 times per week for each of the disciplines of care. Hours of operation are from Monday – Friday, 7am – 4pm. The tool measures overall confidence with 6 activities as reported by the patient. Such confidence is directly correlated with functional improvements.
