Questionnaire: Part A As described in the Randomization presentation, four rice sub-plots should be sampled from each mesh. Section A is for one rice sub-plot only. Therefore, for each mesh, you should fill out four Section A forms. Crop cutting should only be done on rice plots that are ready for harvest.

Mesh Identification and Other Information Questions A1-3. We start with name of the Province and its corresponding code. This is followed by the codes and names of the District and Village. You will have this information from the field validation exercise done previously DOES ANYONE REMEMBER HOW TO LOOK UP ALL THIS INFORMATION ON THE IPAD?

Mesh Identification and Other Information Questions A4-6. Write the mesh/pixel ID of the sample mesh selected, followed by the Center pixel LATITUDE and Center pixel LONGITUDE. You may get the information by tapping any side of the square mesh using the iPad. DOES ANYONE REMEMBER HOW TO LOOK UP ALL THIS INFORMATION ON THE IPAD?

Mesh Identification and Other Information Question A7. Write the Plot ID of the specific plot owned by the farm operator. Write the Plot ID as p01 for plot number 1, p02 for plot number 2, and so on. DOES ANYONE REMEMBER HOW TO LOOK UP ALL THIS INFORMATION ON THE IPAD?

Farm Operator Question A9. Write down the full name of the farm operator. Question A10. Write down the address of the farm operator. This will be critical in tracking down the responder when conducting the follow-up farmer recall survey. Question A11. Write down the phone number of the farm operator. If the farmer has more than one, record all of them.

Questionnaire Respondent Question A12. Write down the full name of the questionnaire respondent. -This is a different person if the OPERATOR is different from the RESPONDENT! -For example, if the operator is unavailable but his wife is, then the respondent is the wife since she is responding to the questionnaire. -If it is the same person as the farm operator, then write down the same name as in Question A9.

Questionnaire Respondent Question A14. Write down the address of the respondent. This will be critical in tracking down the responder when conducting the follow-up farmer recall survey. Question A15. Write down the phone number of the respondent. If the respondent has more than one phone number, record all of them.

Team members/Enumerators Question A16. Names of team members/enumerators. Here the interviewer writes the full names of all members of the team. DO NOT INCLUDE THE TEAM LEADER/SUPERVISOR.

Team member/Enumerator code: -Every team member/enumerator will have a unique code -This is always filled in for each and every questionnaire he/she attends -This number will not change over the course of the survey Enter enumerator codes next to enumerators’ names

Interview Start and End Time Question A18. Time interview start Question A19. Time interview end -The interviewer should record the starting and ending time of the interview. -Times are divided into two boxes, one for the hour, and the other for minutes. -The hours entry box is before the “:”. Date of Interview End -Write the date the interview began, and the date the interview was completed -Start with day, month and then year. -Remember that day is two digit (DD), month (MM) is two digit and year is four digit (YYYY). -Even if the date is 1, enter it as 01. -If the Month is 8 (August), enter it as 08.

Date of the Interview Example: What if the interview began on September 29, 2015, and ended on October 1, 2015? Example: What if the interview began on October 12, 2015, and ended on October 12, 2015?

Questions for the Team Leader only Leave Questions A22, A23, A24, and A25 blank for now, and skip to Section B. THIS SECTION IS FOR THE TEAM LEADER THE TEAM LEADER WILL FILL OUT THESE FIELDS AFTER THE INTERVIEW

Questions for the Team Leader only Questions A22, A23, A24, and A25: Questions A22-A24. These questions are for the team leader to fill out with his/her name, code, and the date of questionnaire inspection. Question A25. Include any relevant views on: -How the interview was conducted -Problems encountered -Etc., which may help the team leader and the data analysts of the results of the survey. Observations should be written at the END OF THE INTERVIEW!
