Basic Medical Insurance maximum benefit of $250,000 for participant; $50,000 for spouse/children. no “co-insurance” hospital room & board at semi-private rate 52 week coverage pre-existing conditions at $25,000 for 1 st twelve months.
Program contains standard U.S. “cost-sharing” features. A “deductible” of $50 per accident or illness. “Usual, customary & reasonable” expenses… Limitations –Semi-private room rate –$250 per tooth for dental Exclusions –Suicide, skydiving or drunk driving, etc.
“AD&D” Accidental death and dismemberment maximum of –$25,000 for participant –$5,000 for spouse – $1000 for child

Using the insurance… Can I go to any doctor or hospital I want? –Yes. The policy is “fee for service”. Will I have to pay when I go to the Doctor? –No. If your Doctor or hospital agrees to accept your IEES card as proof that you are insured. –Yes. If they require payment at the time of service.
How do I get reimbursed? You must file a claim. –Which is a request for payment… Complete the claim form and mail it to IEES WITH –the original itemized bill or –Section B completed.
What about claims incurred outside the U.S.? You must still file a claim. –But the health care provider does not need to use U.S. medical diagnosis codes. –The claim does not need to be on an official claim form If the claim is for less than U.S. $100 the Program Coordinator may use a simplified form. –Mail the form to IEES with original bill or receipt from the doctor.
