Moving Knowledge Instead of People

Project ECHO is an educational and guided practice model that provides access to best-practice information via case review. All information is HIPPA compliant and de-identified.

“One to Many” –Increase access to specialty care for complex conditions Hubs & Spokes - ECHO provides front-line providers with the knowledge and support ECHO links expert specialist teams at a specialty ‘hub’ with care providers in local communities – the ‘spokes’ of the model. Together, they participate in regularly scheduled teleECHO™ clinics ECHO HUB Team of Specialists ECHO SPOKE Providers CLIENT REACH

WHY ECHO? THE UNDERSERVED CLIENTS PROBLEM: Underserved clients have limited access to quality specialist care for common complex conditions such as dementia and intellectual disabilities SOLUTION: A model that offers mentoring and guided practice to build system capacity.

WHY ECHO? Providers PROBLEM: Wants to advance skills and relationships. Lack access to knowledge and training to provide specialty care for individuals with IDD. Often feel socially and professionally isolated. In New Mexico: More than 76k free CME/CE credits issued. SOLUTION: Caregivers engage in a learning community with like-minded providers and specialists within an interprofessional team. Develop specialized knowledge. 1.  ECHO--Extension for Community Healthcare Outcomes is a videoconferencing format through Zoom  FREE--Trinitas partners with University of New Mexico to offer Zoom for free to ECHO participants  Co-management of clients via case based learning  ECHO is a robust method to safely and effectively respond and better manage complex conditions.

1. S-COPE ECHO held two Fridays per month for 1.5 hours focusing on adults 55+ with dementia or behavioral health challenges who reside in LTC

2. CARES ECHO held two Fridays per month for 1.5 hours focusing on adults with disabilities and co- occurring behavioral health challenges

3. GWEP ECHO held one Wednesday per month for 1.5 hours focusing on adults with disabilities who reside in DD Provider agencies and have dementia
