WHY ECHO? THE UNDERSERVED CLIENTS PROBLEM: Underserved clients have limited access to quality specialist care for common complex conditions such as dementia and intellectual disabilities SOLUTION: A model that offers mentoring and guided practice to build system capacity.

WHY ECHO? Providers PROBLEM: Wants to advance skills and relationships. Lack access to knowledge and training to provide specialty care for individuals with IDD. Often feel socially and professionally isolated. In New Mexico: More than 76k free CME/CE credits issued.  ECHO--Extension for Community Healthcare Outcomes is a videoconferencing format through Zoom  FREE--Trinitas partners with University of New Mexico to offer Zoom for free to ECHO participants  Co-management of clients via case based learning  ECHO is a robust method to safely and effectively respond and better manage complex conditions.

1. S-COPE ECHO held two Fridays per month for 1.5 hours focusing on adults 55+ with dementia or behavioral health challenges who reside in LTC

2. CARES ECHO held two Fridays per month for 1.5 hours focusing on adults with disabilities and co- occurring behavioral health challenges

3. GWEP ECHO held one Wednesday per month for 1.5 hours focusing on adults with disabilities who reside in DD Provider agencies and have dementia
