Integrated Digital Care Record Proof of Concept Update - May 2017

Introduction The requirement for a Doncaster Integrated Digital Care Record (IDCR) was clearly identified in the Local Digital Roadmap and the Intermediate Care Case for Change It was agreed that a Proof of Concept for the IDCR would be developed focussed on the Rapid Response Pathway We agreed with our partner organisation that the CCG would take the lead on the procurement of the Proof of Concept solution A budget was secured from the BCF fund and CCG non-recurrent monies Support for the procurement of the system was engaged from Channel 3 Consulting

The Vision for the Integrated Digital Care Record Timely sharing of accurate health and social care information An IDCR solution that can interface with all systems in use in the Doncaster region A shared assessment and plan which will move with the patient A reduction in paper based activities which can ultimately be eliminated Access to the IDCR from any location The option, where appropriate, for patients & carers to have access

Engagement Engagement sessions (group and 1-1) ran January and early February Very positive engagement with clinicians, service managers and technical staff. All keen to support the process and setting ambitious goals for the IDCR “A solution that supports effective care delivery through timely & appropriate access to the holistic health and social care record.. supporting integration, alerting, communication and workflow on any device, at any place and time..”
