Characteristics of Health Activation Solutions Health Activation (patient engagement, population health, personalized medicine, shared accountability, etc.) Characteristics of Health Activation Solutions Dramatically high levels of patient engagement and patient satisfaction Measurable and significant improvements in patient compliance to protocols Measurable and significant improvements in clinical outcomes Substantial increase in case management patient management ratios Measurable and significant reduction in in-person clinical visits (including readmissions) while improving clinical outcomes

Type 2 Diabetes Prevention - Omada Health Activation – Actual Results - Various Healthcare Systems (1 of 6) Type 2 Diabetes Prevention - Omada Initial diabetes prevention program (DPP) enrolled more than 2,000 participants over the age of 65. Contrary to the expectations of many, seniors enrolled in the program have been easily able to integrate the technology elements of a digital diabetes prevention program into their daily routines. Six months after starting the program, 84% of participants, with an average age of 69, remained engaged in the program, and lost an average of 7.8% of their body weight. According to the authors of the original DPP trial, this amount of weight loss in participants 60 or older lowered risk of developing diabetes by 71% over three years. 75% of the people who met their first weight-loss goal kept that weight off for three years. In comparison, only 5% of people who lose weight on traditional diets do the same. Research shows that if you lose just 5% of your weight, your risk of developing type-2 diabetes drops by 58%. Lose 10% or more, and it drops by 85%. To date, over 30,000 people have participated in the Omada program with similar (though lower) results. Additionally, health plans and employers are only charged for those participants who succeed in measurably lowering their risk for type II diabetes by achieving and maintaining a 5% weight loss from baseline. Is the “initial diabete4s prevention program” referenced in red text, Omada’s initial trial that they published? 4th bullet – would change to in measurably…..achieving a 5% weight loss from baseline.

Diabetes – Twine Health Health Activation – Actual Results - Various Healthcare Systems (2 of 6) Diabetes – Twine Health Average 3.2 drop in HbA1c in 3 months for patients starting basal insulin (typical drop is 0.5 to 1.0 in 1 year) Diabetes Treatment Satisfaction Questionnaire demonstrated significant experience improvements Average cost savings per patient estimated to be between $3,000 and $4,000 Hypertension – Twine Health Average 26mmHg drop in blood pressure in 3 months (typical drop is 6mmHg in a year) 100% of intervention subjects achieved target blood pressure < 140/90 in 3 months Primary care visits and associated utilization reduced by more than 50% and case load capacity increased by ~3x Congestive Heart Failure – Sensely 95% of all eligible participants enrolled and used the service continuously 77% of participants compliant with discharge instructions Growing pilot now has over 80 participants with 4% readmissions compared to current baseline 30 day readmission rate of 16% Demonstrated cost savings of over 75% 100% members provided very positive feedback
