Since the last CHILA, we are excited to share what we are most proud of…. Two new community murals! Neighborhood focus on community health work Expansion of the Thriving Weld community data dashboard to all United Way grantees/programs Partnering discussions with Colorado for utilization of the Thriving Weld community data dashboard

A story about something we learned since CHILA1…. During the summer months, NCHA community health workers focused on the neighborhood surrounding the Community Health Clinic. The population is low income, with a high minority rate. The community health workers partnered with the Weld Food Bank to provide free lunch for the families in the neighborhood, and served 1160 lunches over 7 weeks. We surveyed families to better understand their needs and discovered that the #1 request was English classes! The refugees and migrants are motivated to learn the English language, in order to become more self- sufficient.

What we learned about engaging community members with lived experience The NCHA Community Care Corps is a team of community members with lived experience, to better understand and relate to those we serve in the neighborhoods The FQHC has board members with lived experience to better understand the needs of the population served *The NCHA care management team has a give back program. This give individuals a chance to improve self-esteem, feel part of something outside of themselves which gives them a new outlook on life.

What we learned about engaging the BROADER community in developing our aim There are a multitude of choices in our community for an Aim for health improvement. The community is already taking action for a Healthier Weld County! Childhood Obesity Rates – Weld County, CO

Our aim for SCALE is….. DRAFT: By December 2020, Weld County children ages 2-14 living at a healthy body weight will increase by 20%. We plan to ensure that the children of Weld County will have access to healthy nutritious food, and will increase physical activity to improve overall body weight.

Metrics we are considering to track our progress: Metrics to achieve healthy body weight for children Increase physical activity in Weld County schools Provide healthy farm to school meals in all school districts Reduce sugary drink consumption Double SNAP benefits for Farmer’s Market produce Support local community gardens

What we learned from “Switch” Direct the Rider: Study the bright spots and duplicate Motivate the Elephant: Use emotional levers Shape the Path: What looks like a people problem is often a situation problem.  Make it easy to embrace the change. 

Top two things we learned from other communities since CHILA1 HEALTHIER LARAMIE COUNTY Laramie County has a common intake form used by 12 agencies. They took on this great challenge and benefit greatly from all of the hard work. The donated prescription program is very strong and is a remarkable example of what a community can do!

Our biggest challenge(s) right now are….. We have limited, reliable longitudinal data available for childhood obesity at the state or county level We need to attack childhood obesity from infancy, through prenatal outreach It is a challenge to yield a measureable outcome with regard to childhood obesity Challenges our children face – *childhood obesity statistics & facts: Childhood obesity has more than doubled in children and quadrupled in adolescents in the past 30 years. The percentage of children aged 6–11 years in the United States who were obese increased from 7% in 1980 to nearly 18% in 2012. Similarly, the percentage of adolescents aged 12–19 years who were obese increased from 5% to nearly 21% over the same period. In 2012, more than one third of children and adolescents were overweight or obese. *Ogden CL, Carroll MD, Kit BK, Flegal KM.
