Prepare your Application Documents Public Health Sciences http://www.dlsph.utoronto.ca/page/application-instructions-and-forms Scanned Transcripts only (unofficial transcripts will suffice for initial review) Two Letters of Reference (use only institutional email addresses. On paying your online application fee, your referees will receive an email from the University with a link to the reference form, which they will complete and submit online) Curriculum Vitae Letter of Intent (Max two pages – spacing/formatting is up to your discretion) Statistics Requirement form GRE General Test scores (Master of Public Health in Epidemiology ONLY) English Proficiency Test scores (see slide 7)
International Applicants English proficiency must be established by all applicants whose primary language is not English and have graduated from universities outside of Canada For some countries EPT is demonstrated where language of instruction was English. NOT all countries. See sgs.utoronto.ca to be sure. Applications will not be considered without proof of English proficiency An English proficiency test result must be available prior to the application deadline. Test score no more than two years old. Applicants must indicate their EPT score on the on-line application. ALSO, request original test scores be sent, electronically, directly from the testing service to the University of Toronto (Enrolment Services). Your application will not be considered without proof of English language proficiency.

The system will only allow for you to apply to one field of study within a degree program. If you have applied to the wrong field of study, you will need to contact our graduate office to see if this can be amended.
Letters of Reference Contact referees AS EARLY AS POSSIBLE They have 2 weeks to submit reference if their deadline is less than two weeks past the application deadline, this is acceptable, But NO possibility of further extension. Applications without two complete letters submitted are incomplete and not eligible for review.
Once you submit your application fee payment 7) Collaborative Program If interested, select a collaborative program http://www.dlsph.utoronto.ca/program-type/collaborative/ Once you submit your application fee payment you cannot go back and make changes to your profile. You will, however, be able to upload your supporting documents. There is no final submit button for the SGS online application. After the deadline, applications will be reviewed for completion, during this assessment we will waive items that may not be applicable to you. Your application will remain as “docments pending” and move your application to “under review”. Thank you for your patience.
