Documentation Activity – Step 1 Review the last page of the CRNA Break all three (3) goals into S-M-A-R-T framework

Documentation Activity – Step 2 Choose one (1) of the three (3) goals Begin KSR process: Brainstorm a list of things that you may need to accomplish the goal

Documentation Activity – Step 3 Take your brainstorm list and break the items into the knowledge, skills, and resources Complete the check-off boxes

Documentation Activity – Step 4 Prioritize three (3) items from the list Break the three (3) into S-M-A-R-T objectives

Documentation Activity – Step 5 Begin to develop the Individualized Rehabilitation Plan (IRP) Complete the following sections: Rehabilitation goal Valued life role Wellness dimensions KSR Development/Measurable Objective Interventions Make sure the interventions are in S-M-A-R-T format

Documentation Activity – Step 6 Finish completing the IRP: Credential responsible Band Location of service Frequency Duration Band/HCPCS code # of units

Documentation Activity – Step 7 Pass your IRP to the next group Complete an IRP checklist based on another group’s IRP
