What is a titer? A titer is a blood test that measures your immunity to a disease. Get quantitative titers. Upload lab report. For more information visit website.

Immunization Requirements What do I have to submit?

Titer showing immunity. Hepatitis B Titer showing immunity.

MMR (Measles, Mumps, Rubella) Titers showing immunity (for each). 2 vaccinations. OR

Varicella (Chickenpox) Titer showing immunity. 2 vaccinations. OR

Varicella (Chickenpox) NOT History of Chickenpox.

Tdap (not DPT, Dtap, or Td) 1 vaccination. If Tdap is +10 years: OR + Old Tdap Booster Td

Tuberculosis Screening (TB) Initial Screening 2 Skin Tests (PPDs) Within Year.* Chest X-Ray Results. Lab report. OR OR *Prevents false negative.

Tuberculosis Screening (TB) Annual Screening Chest X-Ray Results. Lab report. Skin Test (PPD) OR OR

Free TB Skin Testing Events Do NOT get a skin test if you tested positive in the past! Schedule on website: www.unthsc.edu/immunizations

Only during flu season: October-March Influenza (Flu) 1 vaccination annually. Only during flu season: October-March

Medical History and Physical Examination UNTHSC form is recommended, not required. Documentation submitted must have similar information. 4 pages

Deadline You will not be registered for Spring 2018 until all requirements are met. You do not have to use the Student Health Clinic, but it is recommended. Wait until classes start. Open 7am-4pm Monday-Friday 817-735-5051 StudentHealth@unthsc.edu
