Co-brand logo here Background Welcome When I first walked on the ward in September 2016 I thought that the it should be closed as I felt it was unsafe for patients We set up mini morning MDTs to improve teamwork and communication on the ward Main title for slide set Optional sub title or name

Welcome Benefits to patient and staff Main title for slide set Co-brand logo here Benefits to patient and staff Welcome Patients Length of stay has decreased from 41 to 21 days Feedback Fridays now in place and acted upon Patient co-production in art projects, twitter ‘quotes of the day’, hospital blog Medication omissions decreased from 37 in September 16 to 6 in March 17 Carer involvement improved. Carers Appreciation Day in June 17 Daily patient morning meetings Staff Supervision now at 95-100 %, staff feel more supported and listened too Fully recruited to HCAs and recruited 5 RMNs Weekly bitesize training and staff meeting Monthly risk huddles Monthly Serious Incident learning lessons forum Matron & Ward Manager work a shift on the ward each week Audits completed by ward staff not management so take ownership of work Main title for slide set Optional sub title or name

Ward Away day with staff being asked to reflect on where they were 6 months ago, where they are now and where they want to be. Staff were difficult to engage at first as they had never been asked where they wanted to be in the future. Role modelling and office open door policy is very powerful for both staff and patients.
