Objectives: MyDATA MiSiS Toolkit Student Cumulative Record Foster Youth Counselors will begin to understand how to fill out Comprehensive Academic Assessment form by using the following resources: MyDATA MiSiS Toolkit Student Cumulative Record

Group Activity Work together to complete the Comprehensive Academic Assessment forms as best as you can. There will be missing information and that is Ok. After filling out the form answer the following questions: 1) What information were you able to obtain? 2) What information is missing and how can you obtain it? 3) What might be your challenges?

Review of Comprehensive Academic Assessment

What information were you able to obtain? Student Biographical Information MyDATA and MSIS: Ethnicity, Date of Birth, LAUSD ID number, SSID number, Address and home language. Caregiver Information MiSiS and/or Student Cumulative Record: Court order School Information MyDATA: Last year’s attendance, state testing date and scores, GPA, CAHSEE date and scores, credits earned, suspension, and counselor’s name. School History MyDATA has enrollment years. MiSiS has enter and exit dates.

What information is missing from the student biographical information section, and how can you obtain it? Residency Status Provided to counselors through the DCFS-LAUSD Data Match. Lead Counselor ICP/IGP Information Academic Counselor Court Case Number Social Worker, Educational Rights Holder and/or Student Pending Tickets/Warrants Educational Rights Holder Request form DCFS 1399 from Social Worker Contact DCFS Youth Education Support Department by email If the ERH is not a biological parent, then request form JV-535 from DCFS

Caregiver information What information is missing from the caregiver information section, and how can you obtain it? Caregiver information MiSiS Enrollment forms Biological parent(s) access/contact restriction DCFS Placement papers DCFS form 1399 Student Cumulative Record

What information is missing from the contact information section, and how can you obtain it? Minor’s Attorney Contact caregiver, student, Social Worker, and/or contact Children’s Law Center. Probation Officer Contact caregiver, student and/or Social Worker. Call P.O. Index Phone number County Social Worker and Supervising Social Worker DCFS-LAUSD Data Match Youth Education Support Department from DCFS Child Abuse Hotline (emergency situations) Wrap Around Contact Social Worker, caregiver, ERH and/or student.

Multi-disciplinary Assessment Team (MAT) What information is missing from the student health information section, and how can you obtain it? Multi-disciplinary Assessment Team (MAT) Social Worker Health Information Ask Social Worker, student, caregiver, ERH, school nurse, and/or therapist.

What information is missing from the services information section, and how can you obtain it? COST, SST, Special Education Review student cumulative record Ask counselor and/or teacher Mental Health, Medi-Cal, Oral Health, Vision Ask caregiver, ERH, social worker, student and/or school nurse Tutoring, Employment Training, Youth Source Center, Family Source Center, Homeless Services Ask counselor, student, caregiver, and/or ERH Parent Workshop, WIC/Nutrition, Cal Works/AFLP, Medi-Cal Ask parent and/or social worker

What information is missing from the document information section, and how can you obtain it? Birth certificate Ask ERH student’s personal document Copy can be found in student cumulative record Social Security Card and CA Driver License/ Picture ID Ask ERH, student, and/or Social Worker for student’s personal document/s Transcripts Ask ERH, student, and/or review student cumulative record Request from previous schools of attends Immunization Ask ERH, student, and/or Social Worker School site clinics use California Immunization Registry (CAIR) system

What might be your challenges? Cooperation from parent, caregiver, and/or student. Social Worker or other stakeholders not returning phone calls. Previous school not sending student’s academic, attendance, or behavioral records. MiSiS and/or MyData information not updated.
