What is the 6|18 Initiative? CDC partnering with health care purchasers, payers, and providers to improve health and control health care costs. Targeting six common and costly health conditions – tobacco use, high blood pressure, healthcare-associated infections, asthma, unintended pregnancies, and diabetes… …and currently 16 proven specific interventions that providers, payers and other stakeholders can use to achieve greatest health and cost impact. Aligns evidence-based preventive practices with emerging value-based payment and delivery models.

Learning Objectives Describe the 6|18 initiative including the sixteen evidence-based interventions developed to address the six health conditions Describe the role of providers in implementing the initiative in various practice settings Describe how physicians can utilize evidence-based interventions, collaborate with other payers, health systems, public health and other stakeholders to control high blood pressure Identify the role of a preventive medicine physician utilizing the 6|18 Initiative to transform health system to prevent chronic diseases and provide care with improved quality and coordination
