Patient Navigation Form REVIEW Patient Navigation Form The Patient Navigation Form documents patient’s: Demographic/contact information Hep C medical history Need for referrals Care coordination services Hep C medical care progress Discharge/transition plan The form is an exact replica of the Patient Navigation Database. (3min) (Side 1 of form) PNs can use the Patient Navigation Form as a guide to document patient data and PN services during individual encounters, but must submit monthly data via the database. Each site usually develops their own process for documenting Check Hep C program data. Update the form after each encounter. Keep form in the patient’s chart. Changes: New Checkboxes for health promotion 1 and 2, and Care plan Fields for decline to answer Added transportation Added back patient navigation service level
