WAIUKU – THE DIFFERENCES At Risk Individuals Funding Computerised Cognitive Impairment Pathway Alzheimers Auckland not involved in Assessment

THE PILOT Training 12 months 66 patients referred All GPs involved Practice Nurses did assessments beyond screening Alzheimers Auckland accepted referrals for follow-up The Memory Team provided support

RESULTS: AGES 45-89

RESULTS N=66 MOCA N=44 Dementia N=16 MCI N=10 Uncertain Normal N=8 Nil

RESULTS: ALZHEIMERS AUCKLAND 12 people referred – 3 declined Higher cognitive and EADLs scores than usual Low carer stress No change over 6 months 7 / 9 completed EPOA
