Purpose of the Quality Improvement Approaches Hub Build quality improvement capability and capacity in communities across the US to achieve 100 Million Healthier Lives Specific tools to assess quality in communities Adapt and apply quality improvement methods and tools at the community level

Community Health Improvement Quality Assessment Tool Effort led by Peggy Honore (DHHS) and Rohit Ramaswamy (UNC-Chapel Hill) Convened stakeholder meeting in Spring 2015 to develop tool Adapted from Harvard-funded public health quality assessment tool currently used in mobile health clinics across US Builds off of nine Public Health Aims for Quality (population centered, equitable, proactive, health- promoting, risk-reducing, vigilant, transparent, effective, efficient)

SCALE Initiative

SCALE Initiative Generously supported by the Robert Wood Johnson Foundation 20-month intensive “learning and doing” program for 20 communities working on becoming pacesetters who are paired with an improvement coach and 4 mentor communities. Goal is to support local leaders at every level of a community working together across sectors to improve health, wellbeing, and equity. Learn what it takes to create spread of great approaches between communities to accelerate the pace of improvement.

Pathway to Pacesetter

Pathway to Pacesetter Extends the reach of SCALE beyond the current 24 communities and will provide a more intensive experience than is currently offered through the 100 Million Healthier Lives initiative. The P2P program will offer a menu of options to support communities including access to the following: Free P2P Monthly Webinars, support from 100 Million Healthier Lives, the HealthDoers online learning community platform, and peer community teams; and Two fee-based options: an online certificate program featuring IHI Open School content and/or participation in a Coach- Supported Peer Community Team. Opportunity to attend three in-person meetings
