The Care Home Support Team Holly Stockdale Contract, Risk Management and Quality Policy launch events 9th and 10th August 2016

Background Commissioned by Somerset Clinical Commissioning Group in 2015 To provide support and advice to nursing homes, enabling them to proactively manage residents, make good quality care better, and avoid unnecessary hospital admissions.

The Team Jacqui Cross (Infection Control) Michelle Bell (RGN) Holly Stockdale (SW) Ana Alves (Medicines Management) Charlotte Brown (Safeguarding Lead Nurse and Team Manager)

Infection Control Support Jacqui: Advice to residential and nursing homes in Somerset. Assistance with preventing and controlling infection. Analyses healthcare associated infections relating to care home residents. Participates in the development and production of strategies to prevent future cases. Shares learning using route cause analysis techniques. Provides specialist infection control advice to patients, visitors, and members of the public as required. Investigates Clostridium difficile, and assists with reviews across the community, care home and Primary Care sector.

Nursing support Michelle Support and advice on clinical aspects. Training and revalidation for RGN’s. Liaising with other health professionals.

Social Work support Holly Support and advice with the Mental Capacity Act, Deprivation of Liberty Safeguards, and safeguarding. Liaising with Somerset County Council.

Medicines Management support Prescribing support pharmacists work part-time in GP practices on behalf of NHS Somerset CCG Scope of medicines optimisation in care homes (residential & nursing): Improving patient safety in terms of medicines taking, prescribing, administration and monitoring Resolving medication-related problems and errors Switching medicines to more cost-effective alternatives where appropriate Stopping inappropriate medications (often reducing polypharmacy and its associated risks) Improved recording of allergies where records were missing or incomplete Reduction of medicines waste (nationally, around £50 million/year worth of NHS supplied medicines are disposed of unused by care homes) Improving communication between healthcare professionals involved i.e. care home, GP practice and community pharmacy.
