High Potential – Worker Injured While Resting Under the Vehicle HSE LEARNINGS INCIDENT: Banksman was injured in the legs when he tried to jump out of the way as the driver moved the truck forward. His right ankle hit the front bumper and shoe on the right leg hit the left knee. He suffered contusions. Root Causes: Banksman was sitting in the shade of the truck in front of the right wheel. He was not visible to the driver from the drivers seat. Driver violated the rule that movements of the vehicle on site are controlled by the banksman at all times. Lapse in focus on safety: Both IP and the driver were idling on the site for over 3 hours. As there was no work for them. LEARNING: Inadequate compliance: Movement of vehicles on construction sites must be controlled by a banksman at all times. Drivers are not permitted to move vehicles without a banksman. Inadequate planning, management expectations and priorities: The first line supervisor was not given clear instructions on how to engage idle employees. The driver and the banksman were idle because of a general strike of equipment operators. Key Learning: Idle employees at site get exposed to uncontrolled unsafe conditions. Crew leaders at site must exercise control on movements of people and machinery at all times. Use provided Shade to rest.
