Innovate Integrate Transform SIM is helping Colorado practices integrate behavioral health and primary care and test value-based payment models.

COLORADO SIM Goal Goal: Improve the health of Coloradans by providing access to integrated physical & behavioral health care services in coordinated systems, with value-based payment structures, for 80% of Colorado residents by 2019. Behavioral health addresses issues involving both mental health and substance abuse. Colorado’s unique approach toward integrating behavioral and physical health makes it a national leader in reform.

Opportunity is intended to complement the existing package of practice transformation support In addition to the amazing Technical Assistance and community supports provided in blue via Regional Health Connectors, business consultation services through MGMA, and the awesome work of our Practice Transformation organizations, we understand that financial backing is needed to support change. Practice participation payments offset costs of attending learning collaboratives and reporting on CQMs, but isn’t enough to actually implement plans for integration. Ultimately, we are working toward large-scale broad reform efforts that can help make integrated care in Colorado financially sustainable, but we know it’s not like flipping a switch, reform efforts will take time, support will be varied. In the short term, small grants are intended to provide seed funding to really catalyze getting the systems in place that are needed to support long-term integrated care.
