40. SEPARATION HISTORY AND PHYSICAL EXAMINATION Pre-Separation Counseling 40. SEPARATION HISTORY AND PHYSICAL EXAMINATION A comprehensive medical evaluation, the Separation History and Physical Examination (SHPE), is a requirement for all separating Service members. Ensures any medical conditions requiring ongoing care are identified and documented in your military medical record. Members making a disability claim with the VA are encouraged to begin the process 180 days prior to their separation. Visit TRICARE . GOV for more information

Pre-Separation Counseling …continued It is essential that the member’s Service Treatment Record (STR) (medical and dental records) is maintained with the appropriate DoD authority, such as the member’s medical department or Military Treatment Facility (MTF). You are entitle to access your entire record, and the MHS will increase and improve your on-line access to it. You may also ask for a printed copy. You should review your record as part of your separation exam and help ensure it is complete by STR, as maintained by MHS and provided to VA for all future VA benefits determinations. Missing information may affect future benefits. The form and further instructions on how to complete the SHPE, as well as linkages to VA resources, are available at the TRICARE OnLine Patient portal: https://www.tricareonline.com.

Pre-Separation Counseling 41. Transitional Healthcare Benefit/TRICARE This information is for individuals who are eligible for transitional health care. This is a benefit through the Transitional Assistance Management Program (TAMP). TAMP offers TRICARE-like benefits for a limited time (180 days), which allows time for you and your eligible family members to enroll in other medical insurance programs when you leave active duty. To be eligible for the 180 day transitional medical and dental benefits, you must be: Involuntarily separated from active duty with an SPD Code that designates transitional benefits; or (6) Separated from active duty and agree to become a member of the Selected Reserve, the Ready Reserve or a Reserve Component.

Pre-Separation Counseling Information on Availability of Healthcare Medical coverage can be very expensive, so it is important you are aware of what is available to you and can make the best choice for your own unique situation. The best way to get all of this information is to visit or call a TRICARE representative.

DoD’s inTransition Program Pre-Separation Counseling … DoD’s inTransition Program is a confidential coaching program that answers questions about treatment options, provides information about community resources, and secures an appointment with a behavioral health provider at your new location. All Service members who have seen a behavioral health provider within one year of separation from active duty will be automatically enrolled and will be contacted by an inTransition Coach who will provide you the ability to opt-out of the service. Pre-Separation Counseling
