(Ext. 5470) Medication All medication must be delivered to the nurse in the original prescription bottle or over the counter medication packaging. All medication must be delivered to the nurse by an parent/guardian Students may not carry medication into/out of school or have on their possession at any time. (Examples of exceptions – inhaler or an EpiPen w/doctor’s note) Physician’s notes and medical issues should be communicated directly to Mrs. Madigosky 5 minutes

Medical Services Physicals for all 6th grade students are due by September 30, 2017 Tetanus, diphtheria, and pertussis (Tdap) and meningococcal conjugate vaccines are due by the first day of 7th grade Students will be excluded from school if they do not have documentation regarding these vaccines Please plan accordingly

Attendance Law & Updated Policy Valid excuse notes must be submitted within 3 days of the student’s return to school or the absence will remain unexcused. The following are valid reasons to be excused from school: illness, danger to health from serious exposure, quarantine, recovery from an accident, death of a family member, medical or dentist appointment when necessary, authorized school activities, pre-approved educational travel (limit to 5 days), pre-approved college visits (limit to 2 days per year), pre-approved religious instruction (limit to 36 hours per year), and observance of the student’s religion. Parents may excuse up to 10 illness related absences. A doctor’s note is required for subsequent absences. Truant – 3 cumulative unexcused absences Parents must be notified within 10 days of the 3rd unexcused absence Habitually Truant - 6 or more cumulative unexcused absences The district must refer the child to a school or community based attendance improvement program or Children and Youth Services for services for possible disposition as a dependent child. The district may file a citation with the local magistrate Attendance Improvement Conference – Must be held when a student incurs additional absences after being considered truant (4 or more unexcused absences) Parents and student are invited A plan is developed to improve student’s attendance Check the District website for more information Valid reasons for an excuse

Camp Canadensis

Medical Information Medical support at camp will provide the same level of care and service that our school nurse provides Medical facility staffed 24 hours a day Accommodations for food allergies Peanut free facility

Cost $165 per student Check or money order only Installments due November 17th February 2nd

Parent Chaperones All potential chaperones must acquire the following clearances and deliver them to Ms. Essie Schiller at the Oakmont Building before February 1, 2018: PA State Police Background check FBI Criminal Clearance PA Child Abuse Clearance http://haverford.k12.pa.us/hms All potential chaperones must complete a Camp Canadensis Chaperone Application before February 1, 2018. This application can be obtained from Mrs. D’Avella in the 6th grade office The principal will select chaperones based on program need Chaperone cost, $110

Please Note… Students must commit to attend camp by April 4, 2018 If your child is expressing hesitancy about the Canadensis trip, then please contact Mrs. Hughes Discipline issues may prevent a student from attending the trip Please check your family schedules now to avoid conflicts with the camping trip

Cabins Student guaranteed to be in a cabin with 1 friend 9-12 students total in cabin Parent chaperones in cabin (dads with males, moms with females) Electricity and running water in all cabins

Activities Randomly selected groups of 21-24 students and parent chaperones Students are encouraged to try all activities Examples include: boating, cooperative games, orienteering, swimming, t-shirt art, climbing “the wall,” cooking, pond study, floor hockey, bracelet art

Meals Many selections for breakfast, lunch and dinner Sensitive to all food allergies/ peanut-free facility

Classroom visitation Start off the night by reporting to homeroom 10 minutes Classroom visits “A” day schedule End at 8:23 PM Curriculum HMS website > Academics > 6th grade > 6th grade curriculum Please avoid discussing personal/confidential/individual student information with staff tonight.
