Avoid hospital readmission penalties Primary reason for Unnecessary hospital readmissions - Medication adherence is an issue that many healthcare IT organizations are trying to address. A combination of engagement by family members and care givers along with patient education and identifying inhibitors (such as lack of money to pay for medication, lack of transportation to pick up prescriptions, etc.) are critical factors to helping patients stick to the medication plans their providers prescribe.

Objective Objective here is to develop a cost effective solution to help chronically ill patients remember to take their medications according to their plan and to report back to their prescribing physician if/when they did. Care givers and care coordinators should have the ability to access this information to proactively help patients adhere to their plan.

How does it work Physician feeds the details of the medication (medicine name and time) into WristMom Whenever the patient needs to consume the medicine, the corresponding letter glows and the WristMom vibrates, alerting the patient to take the medication. WristMom demands the patient to indicate if he/she has taken the required medicine by tapping on the glowing letter which eventually turns off the vibration and the glow The care giver/care taker has access to the patient’s progress via the app installed on his/her mobile device. The care taker also receives a notification when the patient misses a dose Doctor has the provision to monitor the patient’s progress online and also receive immediate updates from the care takers in case of an emergency
