ESSI is a 10 year programme (2006-2016) aimed at improving aviation safety in Europe, and for the European citizen worldwide Partnership, with more than 150 organisations Powered by industry and facilitated by EASA Launched on 14 November 2006, EHEST brings together manufacturers, operators, research organisations, regulators, accident investigators and a few military operators from across Europe. EHEST is the helicopter branch of the ESSI, and also the European component of the International Helicopter Safety Team. More than fifty organisations participate in EHEST to date, of which around thirty are actively involved in EHSAT and EHSIT. EHEST is the helicopter component of ESSI and the European branch of IHST

Methodology Analysis of helicopter accidents using the JHSAT method Added HFACS by Wiegmann and Shappell to better understand the circumstances in which pilot errors occur, and to analyse Human Factors in general EHSAT is committed to ensuring that the analysis carried out in Europe is compatible with the work perform in the U.S. and by other analysis teams worldwide, so that the analyses can be consolidated at worldwide level. The methodology therefore was basically inherited from IHST / JHSAT, which itself adapted to helicopters the methodology originally developed in the late nineties by CAST for the analysis of fixed wing commercial air transport accidents. EHSAT decided to also use HFACS by Wiegmann and Shappell to better understand the circumstances in which pilot (and other personnel) errors are made. 7

In order not to interfere with ongoing accident investigation board investigations, only those accidents are analysed where a final investigation report is available. For this purpose Europe is defined as the EASA Member States (27 EU + plus Iceland, Liechtenstein, Norway and Switzerland) Important to note that this initiative combines European wide data. 8

HFACS describes human factors in more detail, according to a multilayered approach.

Discuss the top 5 and give some example of what kind of items are covered under the subject. Data Issues is a special category. It is not related to accident causal or contributory factors, but refers to the available facts and information on an accident. This ‘Inadequate information’ in report felt by the experts of the analysis teams can for example results from the fact that no flight data recorder was available. This category will be left out of the more detailed SPS charts, in order to focus more on accident causal factors. An example Commercial Air Transport scenario During a HEMS mission after the patient had been loaded the helicopter crew decided to continue the mission in deteriorating weather conditions. The decision to continue was taken because an ambulance was waiting to transfer the patient to hospital. During the take-off in poor visibility and falling snow the right front skid of the helicopter struck the surface and as a result it nosed over uncontrollably and impacted the ground.

The pilot heard a loud noise and the helicopter started yawing. The helicopter landed heavily and sustained substantial damage. The pilot and two firemen received minor injuries.

An example General Aviation - Training scenario The dual exercise was for the student to practise emergency and autorotational landings. The landing area selected for the exercise was muddy with a forecast wind speed of 26 kts. As part of the exercise the flight instructor simulated an engine failure without any prior warning. During the subsequent autorotation the instructor allowed the rotor RPM to drop below the minimum. The helicopter contacted the ground with a high sink rate and rolled over. Specialist Team SMS/Operations Consolidated recommendations: SMS: Encourage the use of SMS based on real safety culture including risk management and codes of practice. SOPs: Operators should be encouraged to establish and apply SOPs for all activities that they undertake. RISK ASSESSMENT/PRE-FLIGHT PREPARATION: Emphasise the importance of Risk Assessment in mission planning
