Living Benefits Think about it: Universal Life does more than pay a death benefit to your beneficiaries. It provides a Living Benefit for Convalescent Care that you can use during your lifetime. The Convalescent Care Benefit supplements the cost of home healthcare, assisted living, adult day care and long term care facility confinement. After 90 days of long term care confinement or services, your Convalescent Care Benefit helps supplement the cost of home healthcare, assisted living, adult day care and long term care facility confinement. 5 If you enroll at or after age 57, your higher death benefit will continue for 14 years before it reduces.

Long Term Care Convalescent Care Benefit Pays up to 4% of your death benefit per month for : Home Health Care Assisted Living Long Term Care Facility Confinement Adult Day Care Payments reduce the death benefit. There is a 90-day elimination period before benefit can be paid. Once benefits begin, they cover the period retroactive to the first day you received care. Total benefits paid are not to exceed the death benefit amount. To receive this benefit you must meet Conditions for Eligibility for Benefits. Receipt of accelerated death benefits may affect eligibility for public assistance programs, and may be taxable. The Convalescent Care Benefit provides limited benefits health insurance only. It does NOT provide basic hospital, basic medical or major medical insurance as defined by the New York State Insurance Department. The expected benefit ratio for this Rider is 68.3 percent. This ratio is the portion of future premiums which the company expects to return as benefits, when averaged over all people with this policy.

What Other Benefits Are Available? • Accidental Death – Doubles your death benefit if death occurs by accident • Waiver of Premium – Waives your premiums if you become totally disabled • Children’s Term – Covers children, from newborn to 23 years old • Terminal Illness Benefit – Advances 25% to 75% of your death benefit if your doctor determines that your life expectancy is 12 months or less
