Waiver of premium for critical illness Waives premium payments for all covered persons for six months when employee is diagnosed and receiving a benefit for a covered illness payable at the 100% or 50% benefit level. Waiver can be activated only once per calendar year. WPC

Waiver of premium for disability Waives premium payments for all covered persons if a doctor determines the employee is totally disabled for six months before age 70. WPD

Healthy Living Rider $50 routine services benefit for early detection and prevention help you stay well such as mammography, colonoscopy, etc.* Covered screenings include: • Mammography • Pap smear • Flexible sigmoidoscopy • Hemoccult stool analysis • Colonoscopy • Prostate specific antigen (PSA) test for prostate cancer • Doppler screening of carotid arteries • EKG/ECG • CT colonography • Human Papillomavirus vaccination (HPV) • CA 125 blood test • Whole body skin cancer screening exam $50 follow-up diagnostic tests to investigate possible cancer, coronary artery disease or cerebral vascular disease *Calendar-year benefits. Healthy Living Rider Genetic testing following a physician’s recommendation because you are at high risk of, or have been diagnosed with cancer, to assess risk for BRCA-1 or BRCA-2 one $250 benefit is payable once per person when the genetic test is performed within 12 months of the recommendation if you have a marker, the benefit doubles to $500 to help pay for additional counseling *One-time benefit per person are payable after the effective date and waiting period, if applicable. Healthy Living Rider Healthy Living Advantage program powered by Activities support you in achieving your personal goals for healthy living with one-on-one interaction and web-based tools. You and your covered spouse receive: • access to the Healthy Living Advantage online portal • health assessment tool • call with an advisor upon completing the health assessment • automatic payment of the $25 biometric benefit when portal is used and to-do’s are completed *Calendar-year benefits. HLR-HLA, (currently not available)

Caregiver Benefit $500 benefit is payable to the covered person (over age 18) who provides care to an eligible family member, whether or not they are covered under the policy, who has cancer, coronary artery disease or cerebral vascular disease Access to medical experts through Best Doctors®, You will have access to a Personal Care Manager, who will work with you to answer your questions and explain your options for the specific illness your loved one is facing.
